Power list 2025
I was featured on “The Pathologist” Power List 2025: Leading Voices Edition.
It is a privilege to be counted among those considered as “inspirational figures behind innovation and achievement in pathology and laboratory medicine”
It didn’t stop there; I was voted one of the readers’ favourites, and I am grateful to you all!
My interview has just been published:

Part 2: Strengthening the evidence on coeliac histology 🔬
How can studies of agreement between pathologists become more useful for everyday practice? This follow-up explores study design, real-world case selection and consistent reporting—and what better reproducibility studies could look like.

🧬 Mucinous carcinoma of the breast: more than just “mucin”!
This review by the authors - Emad A Rakha, Bara Wazwaz, and Stephen B Fox, published in Journal of Clinical Pathology explores the morphological spectrum, diagnostic pitfalls and classification challenges of mucinous breast carcinoma.
🔬 From classical, hypocellular MBC to hypercellular, papillary and micropapillary patterns—the morphology reflects distinct pathogenetic pathways and variable biological behaviour.
The key message for pathologists: recognising mucin is only the beginning. Architecture, lineage, precursor lesions and molecular context matter.
A valuable review for anyone reporting breast pathology.

Trump: I'm the only president to pass. I passed 3 cognitive tests. And they're hard. They have a lion, a bear, a snake, and a camel. They say, which one is the snake? It's a very powerful test, very highly respected test, very well known test, 30 questions, very hard. I passed it all 3 times. I aced it

How reproducible is the histological diagnosis of coeliac disease? 🔬
Feghali, Baby and colleagues reviewed 23 studies and found wide variation in agreement between pathologists. Agreement fell further when biopsies were assigned detailed histological grades.
The takeaway: integrate histology with serology and clinical information. Biopsy quality, sampling and pathologist expertise matter.

Are our pathology guidelines keeping pace with reality?
📖 A thought-provoking proposal to make diagnostic criteria practical, accessible and globally applicable in this excellent paper by the authors - Philippe Benjamin Stephenson, Olaleke Folaranmi, José Jiménez Heffernan, and Sanjay Mukhopadhyay, published in the BMJ Journal of Clinical Pathology.
Three powerful messages:
🔹 Make criteria feasible across resource settings
🔹 Focus on what truly impacts diagnosis and treatment
🔹 Guideline panels should include community pathologists and represent the global community of pathologists
The best guidelines should not just define what is ideal—they should help every pathologist make the best possible diagnosis with the resources available.

Breast - Micropapillary carcinoma can show a distinctive HER2 IHC pattern
- Basolateral or lateral, U-shaped membrane staining
- Score as equivocal (2+) when staining is moderate to intense but incomplete (in >10% of tumor cells) → reflex to FISH
- This pattern can be seen in some micropapillary cancers that are HER2 gene amplified (CAP Breast Biomarker Protocol, 2026).

HER2 testing in the antibody–drug conjugate era 🔬
Liu and colleagues summarise the 2024 Chinese guidelines for HER2 testing in breast cancer, with a focus on reliable assessment of low HER2 expression.
Key themes include careful specimen handling, gradient controls, review of results near scoring thresholds, and reassessment of recurrent or metastatic lesions. The guidelines recommend reporting the IHC score and ISH result, with HER2-low and HER2-ultralow treated as therapeutic classifications rather than labels in pathology reports.

🔬 When a T-cell lymphoma expresses CD20… think twice!
An exceedingly rare case of CD20-positive primary cutaneous T-cell lymphoma, NOS, with an aggressive clinical course.
The diagnostic challenge? True coexpression of CD3 and CD20—a finding that can easily raise the possibility of a B-cell lymphoma. T-cell receptor clonality and the overall clinicopathological picture were crucial in establishing the diagnosis.
A striking reminder that aberrant immunophenotypes can challenge even our most familiar diagnostic algorithms.
