Cardiovascular disease burden has declined across Europe, but important sex-specific inequalities persist Our analysis of 55 ESC countries from 1990 to 2023 shows substantial reductions in cardiovascular mortality in both women and men. However, these improvements mask distinct patterns of inequality: men continue to experience more than twice the rate of premature cardiovascular mortality, while women show substantially greater variation in cardiovascular disease burden between countries. 🫀From Healthy Atria to Atrial Failure: Rethinking Atrial Cardiomyopathy (AtCM) from the #HFA_ESC clinical consensus statement: 📍AtCM extends beyond #AF: Electrical, mechanical & structural abnormalities may precede clinically apparent AF or #HeartFailure 📍Diagnosis: Electrical atrial dysfunction (P-wave score ≥1) PLUS ≥1 of: mechanical dysfunction, atrial enlargement or excessive atrial fibrosis 📍Proposed diagnostic thresholds: - P-wave ≥120 ms (standard ECG) or ≥150 ms (amplified ECG) - Advanced interatrial block: P-wave ≥120 ms + biphasic morphology in ≥2 inferior leads - LA reservoir strain <23% - LAVi >40 mL/m² (>48 mL/m² in women >65 years) - Atrial fibrosis: thresholds pending validation 📍New P-wave scoring system (0–4): Ranges from normal P-wave (0), prolonged P-wave (1), advanced interatrial block without (2) or with paroxysmal arrhythmia (3), to persistent atrial arrhythmia (4). 📍Atrial failure: End-stage AtCM, characterized by persistent AF or HF with symptoms attributable to atrial disease. 📍Early recognition matters! Risk factor & comorbidity management may help prevent or delay progression to atrial failure. Read more in #EJHF @EJHFEiC 🔗 doi.org/10.1002/ejhf.3782 @escardio @h_arfsten @GTersalvi @jozinetm

Chasing gains. Putting the heart at risk. The effects of anabolic steroids can outlast their use - and the cardiovascular consequences can be devastating. 🎥 In “The Price of Performance: Doping and the Athlete’s Heart,” Prof. @SSharmacardio explores how anabolic steroids reshape the heart, why recovery takes time, and the dangers of prolonged use. Do oesophageal temperature probes actually make left atrial ablation safer and are we still using them? That’s the question behind the new EHRA Scientific Initiatives Committee survey on the contemporary use of oesophageal temperature probes (ETPs) during left atrial ablation and the subject of our latest #EHRA_SICsays conversation with @AriSultanEP. 232 physicians from 37 countries responded and the picture is far from uniform. ETPs are used predominantly during thermal ablation, while their use with pulsed field ablation is uncommon. Even when probes are used, temperature thresholds and the response to temperature excursions vary considerably between centres. The key issue isn’t simply whether we monitor temperature. It’s what that monitoring actually achieves. Current randomized and observational evidence has not established that ETPs prevent oesophageal injury or atrio-oesophageal fistula, and 56% cited lack of evidence for AEF prevention as a reason for not using them. Meanwhile, the growing use of PFA is reshaping oesophageal protection strategies altogether. 🎙️ Watch the discussion with @AriSultanEP 👇 📄 Read the full manuscript: bit.ly/4hGfaXZ Does oesophageal temperature monitoring still have a place in your left atrial ablation workflow? #EHRA_SICsays #EHRA_ESC #EPeeps #AtrialFibrillation #CatheterAblation #PulsedFieldAblation #Electrophysiology @LauraPerotta @JarkkoKarvonen @jongichun @micaela_ebert @DrCherylTeresEP @HaranBurri @veramasloo @drtahasahin @AugustoMeretta @JacopoImberti @cespon_maria @jadczyk @Fede79Miglio @CarolRemme @LA_Stanciulescu @VerenaTscholl @Amar__taha @mattans5 @AmrAbdin10 @JuliaCTourigny @DrMarkMills @RachelterBekke @FraSantoroMD @Pierre_Olt @cris_lozano @LinaMarcantoni @ChristianHeeger
