Phase II REPRINT: Enfortumab Vedotin Plus Pembrolizumab in Rare Renal Carcinomas
At the ESMO Congress 2026, Giuseppe Procopio presented preliminary first-stage results from the Phase II REPRINT trial (NCT06302569), evaluating enfortumab vedotin plus pembrolizumab in patients with collecting duct carcinoma (CDC) and renal medullary carcinoma (RMC).
Key findings:
▫️ Study population: 13 patients enrolled; nine evaluable after central pathological review (seven CDC and two RCCU-MP).
▫️ Objective response rate: 22% (2/9), with both responses occurring in patients with SMARCB1-deficient RCCU-MP. No objective responses were observed in CDC (0/7).
▫️ Disease control rate: 55% (5/9), including two partial responses and three cases of stable disease.
▫️ Median PFS: 5.2 months (95% CI 3.4–not reached).
▫️ Safety: No Grade 3–4 adverse events were reported.
▫️ Biomarkers: Both responding tumours were negative for membranous NECTIN-4 expression.
Takeaway: REPRINT demonstrated preliminary antitumour activity and met the prespecified criteria to proceed beyond the first stage of Simon’s two-stage design. The findings are encouraging in the small RCCU-MP subgroup, but further evaluation is needed to establish efficacy and safety.

Phase III LITESPARK-011: Health-Related Quality of Life and Clinical Outcomes in Advanced RCC
At ESMO Congress 2026, Daniel Heng presented a post hoc analysis of LITESPARK-011 (3595RO), examining associations between health-related quality of life (HRQoL), progression-free survival (PFS), and overall survival (OS) in patients with advanced clear cell renal cell carcinoma following anti–PD-(L)1 therapy.
Key findings:
▫️ 741 patients were included in the exploratory analysis.
▫️ Baseline HRQoL: Higher FKSI-DRS scores were associated with more favourable PFS (HR 0.86; 95% CI 0.81–0.91) and OS (HR 0.81; 95% CI 0.77–0.86).
▫️ Longitudinal HRQoL: Higher FKSI-DRS scores over time were associated with more favourable PFS (HR 0.82; 95% CI 0.76–0.89) and OS (HR 0.57; 95% CI 0.52–0.62).
▫️ Landmark analyses: OS hazard-ratio estimates generally favoured stable or improved HRQoL at Weeks 13, 25, and 53, while PFS associations were less consistent.
Takeaway: Higher baseline HRQoL scores and favourable HRQoL changes during treatment were generally associated with better clinical outcomes, particularly OS. These exploratory findings demonstrate associations and do not establish causation.

RAMPART: Quality-of-Life Outcomes With Adjuvant Durvalumab in Resected RCC
At ESMO Congress 2026, Sophie Merrick presented patient-reported outcomes from the RAMPART trial (3590RO), comparing adjuvant durvalumab with active monitoring in 218 patients with resected renal cell carcinoma.
Key findings:
▫️ Primary QoL endpoint: No evidence of a difference in overall health and quality of life (OHQL) between groups at Month 15.
▫️ Week 16: Durvalumab was associated with clinically meaningful worsening in OHQL (−7.4), role functioning (−7.1), cognitive functioning (−5.3), fatigue (+5.5), and appetite loss (+5.8).
▫️ Month 15: Differences persisted in cognitive functioning (−7.1) and appetite loss (+7.4), with later worsening in dyspnoea (+7.5) and constipation (+12.2).
Takeaway: Although the primary QoL endpoint showed no evidence of a difference, exploratory domain-specific analyses identified clinically meaningful effects during and after adjuvant durvalumab treatment. These findings require cautious interpretation, particularly the single-item dyspnoea and constipation outcomes.

Phase III KEYNOTE-564: Final Analysis of Adjuvant Pembrolizumab in Clear Cell RCC
At ESMO 2026, Toni K. Choueiri presented the final protocol-prespecified analysis of the Phase III KEYNOTE-564 trial (3589RO), evaluating adjuvant pembrolizumab versus placebo in 994 patients with ccRCC at increased risk of recurrence following surgery.
Key findings:
▫️ Median follow-up: 85.9 months.
▫️ 7-year DFS: 57.2% vs 44.6% (HR 0.68; 95% CI 0.56–0.82).
▫️ 7-year OS: 82.3% vs 76.6% (HR 0.70; 95% CI 0.53–0.92).
▫️ Subgroup analyses: DFS and OS findings remained consistent with earlier reports across key risk groups and sarcomatoid features.
▫️ Safety: No new safety signals or serious treatment-related adverse events were reported.
Takeaway: After approximately seven years of follow-up, adjuvant pembrolizumab maintained its previously established DFS and OS benefits, reinforcing its role as a standard of care for patients with ccRCC at increased risk of recurrence after nephrectomy.

Phase III LITESPARK-012: Belzutifan or Quavonlimab Plus Pembrolizumab and Lenvatinib in Advanced RCC
At ESMO 2026, Toni K. Choueiri presented results from the Phase III LITESPARK-012 trial (3586O), evaluating two investigational combinations versus pembrolizumab plus lenvatinib in previously untreated advanced clear cell RCC.
Key findings:
▫️ Median PFS: 20.8 months with belzutifan + pembrolizumab + lenvatinib, 20.7 months with MK-1308A + lenvatinib, versus 23.7 months with control.
▫️ Overall survival: Neither investigational regimen demonstrated a significant improvement.
▫️ ORR: 69.1% and 69.0% versus 76.3% with control.
▫️ Adverse-event-related drug discontinuation: 51.6% and 51.4% versus 36.2%.
Takeaway: Adding belzutifan or quavonlimab did not improve efficacy. Pembrolizumab plus lenvatinib remains a first-line standard-of-care option for advanced clear cell RCC.
