🚀 Initial Clinical Experience with Upright Image-Guided Proton Therapy for Pediatric CNS, Pericranial, and Ocular Malignancies

🔬 A single-institution study evaluated gantry-less, image-guided upright proton therapy in 10 pediatric patients (aged 2–18 years) with CNS, pericranial, and ocular malignancies.
📊 Key findings:
✅ All 10 patients completed treatment without interruption.
🎯 Mean setup corrections remained within ±5 mm (translational) and ±2° (rotational), supporting reproducible positioning.
📈 At 3 months:
• Local control in all evaluable patients
•No early disease progression was observed within the treated volumes
•5 complete metabolic
responses, 1 partial metabolic
response
•4 patients with stable disease
🩺 Only one grade ≥3 acute toxicity was reported
💡 Upright, gantry-less proton therapy appears feasible in complex pediatric cases, including craniospinal irradiation and re-irradiation.
🔎 Larger studies and longer follow-up are needed to define generalizability and uncommon complications.

How do AI and 🧑⚕️👩⚕️ compare in contouring and planning #bcsm APBI cases?
In this single-institution feasibility study, AI-generated APBI contours and plans were compared to those generated by 👩⚕️🧑⚕️.
🔘 AI and physician contours demonstrated 🔼 agreement: GTV Dice = 0.78 and PTV Dice = 0.83
🔘 When comparing AI autoplanned cases to those generated/delivered by 👩⚕️🧑⚕️: AI had 🔽 % ipsilateral lung V5 and contra breast Dmax, but 🔼 % heart V3
🔘 When combining AI contouring + autoplanning: 6/7 plans met all institutional DVH constraints with similar PTV D95% to 🧑⚕️👩⚕️ plans
