Other anaesthetists to me: “What’s with this #GastricUltrasound stuff you keep going on about? Never needed it. It’s all hocus #POCUS. Just stick the tube down. Are you a man or a mouse?”

The Currency of the Journal Article Is Collapsing
As an editor I can say that we are being flooded with AI-assisted work.

Lots of debate this week on whether journals should ban AI in peer review.
We actually tested this in our new preprint, and the results were pretty clear to me.
For context, I've reviewed for most of the big journals and evaluated 1,000 human reviews as an editor. I love to read and evaluate science.
TLDR: From now on I won't do any peer review without AI. For clinical trials, I'd even call human-only review dangerous 🧵

Great to see a specialist college publishing psychometric data on their single-moment-in-time high-stakes examinations.
#SMITHSEx. Well done @CICMANZ

Paying peer reviewers = 85% faster editorial decisions.
And payments gave high-quality reviews.
(the story is described in Nature)
At Biology Open, peer reviewers were paid for high quality reviews.
$290 per peer review.
Miraculously, the time to editorial decisions DROPPED:
- Unpaid reviews: 38 days.
- Paid reviews: 5-6 days.
And review quality WENT UP.
📍How it works:
“When researchers apply to become paid reviewers for Biology Open, we screen their CVs and check whether they’re legitimate and have peer-review experience. We currently have 500 pre-contracted reviewers from all over the world. Ideally, what I envision is a system where reviewer applicants undertake a manuscript-reviewing test. And we’d review that review, so if it scored high enough, we’d hire them.” - Editor-in-chief Daniel Gorelick.
Nature: “Do you think paying reviewers could work in other journals and in other disciplines?”
Editor-in-chief: “Absolutely. There’s been such a profound aversion to paying peer reviewers, whether it’s on principle or because people think it will never work. We’ve demonstrated that it works.”
So...
At last, there’s light at the end of the tunnel.

Do we need a video on an SGA (routinely)
I was struck by the fact in a poster I was down to judge that there was no correlation between laryngeal view down an I-gel and position over the larynx
What is our goal?
Good ventilation or perfect positioning over the larynx?
I post the question cos I don’t know the answer. But we should consider it before attaching a camera to everything just because we can.
Next someone will say we need to store a video of the airway through every anaesthetic ..as our data centres burn the world up!
