Myth: a score of 4 or more on the 4AT means the person has delirium. Fact: a score of 4 or more suggests delirium and should lead to a clinical assessment. A clinician makes the diagnosis, using judgement and the whole picture. People with severe dementia can score 4 without delirium, because they may be unable to do the age and months questions. Staff then look at whether there has been a recent change.

Ten years ago, I couldn’t walk, speak, or scratch my own face. Today, I spoke at Johns Hopkins about the extraordinary people who helped me reclaim my life. Rehabilitation is more than movement. It’s dignity, connection, and freedom. Grateful for @KellyCasey_OT @DrDaleNeedham @icurehab
Great oral abstract presentation by @SGFesquire at @icurehab conference A vivid discussion of the patient perspective of severe critical illness including delirium & mobility

I was unable to return to work following my #ARDS/#ECMO event and as a single parent to three children and the only income stream into my household, I was placed in a financial straitjacket from which I am yet to emerge. An under-recognised aspect of #PICS
Now speaking is Ann Parker MD PhD One of her topics is financial toxicity after ICU part of @icurehab conference Ping @HopkinsPCCM

Ten years ago, I was just a few weeks removed from this photograph. I couldn’t walk, speak, or scratch my own face. Tomorrow at Johns Hopkins, I’ll speak about what happens after survival—and the extraordinary people who help us reclaim our lives. Rehabilitation is more than movement. It is dignity, humanity, and hope. Grateful for @DrDaleNeedham @icurehab @KellyCasey_OT My goal: Making the Invisible Visible. 🫁 Extubation is a milestone, but does it mean recovery? A new article draws attention to an often-overlooked concern: frailty in post-extubated patients. 🔗 ow.ly/EmP05103fYF #Frailty #ICURecovery #EvidenceBasedPractice #PatientOutcomes

One study in Newcastle, England, linked delirium in hospital with faster decline in memory and thinking. Hospital admission without delirium was not linked with faster decline. 205 older people already in a local research study were recruited when admitted to hospital. 82 of them (40%) had delirium. A year later, 135 were retested, along with 100 people matched on age, sex and education who had not been in hospital. Those who had delirium declined faster than those who did not. This was a small study from one area (Age and Ageing, 2021), so it does not settle whether a hospital stay alone can affect thinking. doi.org/10.1093/ageing/afab0…
