Honestly Iāve hit my breaking point. On call this week and just crushed. Itās insanity to do a specialist consult on a complex patient in 20 minutes. If I take longer then urgent care is delayed and the pile gets bigger.
Now Sechelt hospital has no physician capacity and patients are being sent over to me in the night. For the first time in 22 years of medical practice I said no. It felt terrible because the patient gets caught in the middle.
Itās unsafe. Itās hurting me. Itās hurting my family. Itās hurting patients. Leadership at the highest level in the VCH health authority need to lead. I have dedicated 22 years to Lions gate hospital. Iām seriously contemplating resigning.
Whoever forms the next government needs to make people in highly paid positions in administration accountable. Done with mediocrity. Politicians canāt fix the system but they can demand accountability from those they hire in to these roles.

As Canada awaits the Alberta separation referendum results, letās remember how stupid this all is. Huge public money spent on a Russian & American-fuelled separation movement for an awfully-written and badly managed unbinding referendum on whether to hold a binding referendum that would be illegal.

1) Sotatercept is amazing effective for PAH but effects on the RV have been inconsistent. @CircAHA we show that RV contractility truly decreases after sotatercept with no change in RV PA coupling -> the surrogates weāve chased for years with vasodilators

Presented today and published in NEJM, our PADN-HF-PH trial in which 264 pts with left-heart failure (HFrEF or HFpEF) and pulmonary hypertension (mPAP >20 mmHg and PCWP >15 mmHg) were randomized at 25 centers to pulmonary artery denervation (PADN) + GDMT vs GDMT alone.

A study with almost 74,000 subjects ages 50-85.
Conclusion:
-The wealthiest Americans had approximately the same survival rates as the poorest people in Europe.
Wealth helps enormously within the United States, but even high wealth does not completely overcome the USāEuropean mortality disadvantage.
Wealthy Americans presumably have much greater ability to buy excellent medical care, better housing, healthier food, gyms, preventive testing and so forth. Yet their survival remained roughly comparable to the poorest group in Northern/Western Europe.
The study doesnāt provide direct answers to why this happens. However, maybe wealth cannot outperform lifestyle, strong social safety net, great food around friends and family, lower stress, walking significantly more everyday to your bakery (yes, believe it or not, Europeans are big on bread), local food stores, coffee shops, affording to retire when it is time without worrying about paying bills especially when you get sickā¦
As both an American and European citizen, I believe it may be time for Americans to recalibrate how we think about longevity.
We are increasingly chasing the latest intervention such as rapamycin, peptides, cold plunges, red-light therapyā¦hoping that targeting a single enzyme or metabolic pathway will move the needle.
As a systems physiologist, I struggle with that view. Human physiology is not a collection of isolated pathways. Trillions of cells communicate across metabolic, vascular, endocrine, immune and neurological systems. Health emerges from their integration.
Perhaps longevity is less about hacking individual pathways and more about creating the environment in which the whole organism can thrive.
