Milrinone versus Epinephrine After Cardiac Surgery: Does the Choice Matter? An interesting retrospective, entropy-balanced cohort study published in JCVA compared postoperative outcomes in 1,310 cardiac surgery patients receiving milrinone (n = 312) or epinephrine (n = 998) as the initial inotrope. Key findings: No statistically significant difference in 90-day mortality (7.1% vs 6.6%). Milrinone was associated with higher rates of acute kidney injury (73% vs 61%) and renal replacement therapy (13% vs 7.6%). ICU and hospital stays and time in postoperative organ dysfunction were longer with milrinone. However, these findings warrant cautious interpretation. The retrospective design, potential confounding by indication, unavailable intraoperative variables, and differences in treatment duration limit causal interpretation. The findings raise important questions but do not establish the superiority of either inotrope. Randomized trials are needed. Peiris et al.
