He chose a future in which one person dies so that everyone else lives, and he did so with full knowledge of the cost, that is a cold utilitarian calculation made by someone who can literally see the alternative (total extinction or Thanos winning in every other branch). The discomfort comes from the fact that Strange had already decided Tony was the acceptable sacrifice before Tony himself chose to make it.
