#MIS TTC fusion technique tip: correcting valgus deformity. chronic neuropathic ankle fx. The lateral plafond had been eroded and prevented reduction. Severe neuropathy to mid calf. A1c 10. Limb threatening condition and progressive deformity in ex fix. #orthotwitter 1/n
The answer here was a medial mal osteotomy, above the level of the medial mal fracture. By angling obliquely we were able to let the medial plafond shift out of the way and the talus then sat nicely under and inline with the tibia. Now ready for the nail! 4/4
Coming soon: confirming joint prep MIS! #BurrTime
More #MIS teaching here. So you’ve done a MIS bunion, but still need a little bit more? Maybe a lateral release? But how do you do it?
#orthotwitter
Here is what I do. I use a 6400 beaver blade.
1. Mark the dorsal lateral joint.

7. Once the release is compete the lateral sesamoid should not move when the toe is gently pulled into varus.
also, when you dorsiflex the ankle the toe should stay straight and not pull laterally.
