Cocaine has overtaken fentanyl as the drug most often involved in Cuyahoga County, Ohio, overdose deaths for a second year. County projections for 2026 put cocaine involvement at 200 of 340 deaths and fentanyl at 130, but categories overlap because many deaths involve multiple drugs. Overdose prevention needs to reflect a changing local drug supply, including risks to people who may not expect opioids in cocaine.

The DEA has issued a notice of intent addressing 7-OH and mitragynine pseudoindoxyl. Proposed controls include concentrations above 1 mg/g in solids or 1 mg/mL in liquids, or a mitragynine-to-target-compound mass ratio below 100. A new order could be published on or after November 13, 2026. The October notice itself does not enact that order. Existing temporary controls on mitragynine pseudoindoxyl remain effective pending replacement or expiration.
