Whole blood before hospital arrival saves lives.
Prehospital whole blood (PHWB):
• ↑ Hemodynamic stability on arrival
• ↓ 4-hr transfusion needs
• ↓ 24-hr mortality (OR 0.35)
• ↓ cardiac arrest (OR 0.24)
Resuscitation starts at the point of injury.

This study offers details of patients who require ultra-massive transfusion including factors like initial lactate, heart rate, & need for extremity exploration among survivors which may help guide the care of this challenging patient population.

Dr. Randi Smith, Vice Chair of Diversity, Equity, and Inclusion, at #ASC2025 this week! “Always remember our true north!” #EmorySurgery @RandiSmithMD
Amazing panel at #ASC2025 about the importance of surgeon #advocacy to defend against the #DEI backlash. Many sidebar questions after we concluded about how to engage elected officials, so I’m bumping this thread. Thanks @AcademicSurgery & @UnivSurg for this timely session.
Check out our recent work on pREBOA and blood product utilization
pREBOA, when deployed as a resuscitative adjunct deceases blood utilization.

Decision of when to stop in ultra-massive transfusion resuscitation is not based exclusively on units transfused, but rather the integration of patient & time-specific data, supported through machine learning predictive modeling

Excellent presentation (and arguably best title of the day 😂) by @EmorySurgery’s resident @courtneymeyerMD this morning! - “DON’T LEAVE OUT THE LADIES: The safety profile of empire transfusion of whole blood at a single institution” #EAST2024 – at Florida, USA
Thank you @EAST_TRAUMA for a great time and the opportunity to share our work in the Raymond H. Alexander competition. Don’t leave out the ladies! Our results support exceedingly low rates of alloimmunozation in females following LTOWB transfusion @EmorySurgery @RandiSmithMD – at Orlando, FL

Half of all gun homicides in the US took place in 127 cities, which home less than 25% of the US population.
Thank you @RandiSmithMD for this incredible lecture highlighting the important role community violence intervention programs play in public health. #ACSCC23
While there is currently no consensus definition for ultra-massive transfusion in the literature, our review supports the feasibility of defining UMT as 20 or more units of red blood cell product in 24hrs 🩸 @zach_grady @RandiSmithMD @AmJSurgery
