A few months ago, the National EMS Advisory Council (NEMSAC) adopted a proposal calling for the establishment of Paramedic Practitioners. Though a public document, finding it online is difficult. Here it is in full: drive.google.com/file/d/10Gg…

I went on a 17 hour Search and Rescue op in the rain yesterday. 12 hours in, the team was complaining about being "gassed" discussing giving up and I was just like 🤷🏼‍♂️ I'm at like 95% and I've been up since 0230. Could not relate at all. Couple thoughts here... 🧵

Only the most recent time that @gallsconnect has sent me an unhemmed pair of pants. The other pants in the same order were hemmed, as ordered. Over the many years that my employers have outfitted me from Galls, this has happened many many times.

To address the Hospital #WallTimeDelay for @BCFREMS, we implemented a method for our EMTs and Paramedics to report the delay in real time. Once they submit the delay, a text message is sent to my admin staff and the hospital executives (not the ED staff). What Have We Learned? 1. The hospital administrators do indeed care about wall times. When they get the text, they walk down to the ED. 2. The ED staff DOES NOT like this new text alert process because it alerts the C-Suite. 3. Our personnel love it and are very appreciative. NB: I'm privileged to work with an amazing EMS Chief, @EMSOfficer, who follows up on every single text, no matter the time. As you can see from the graph, he was pretty busy last night. This is #TrueLeadership. I would love to hear any other ideas you have about limiting wall times.

Underrated feature of *living in an organized society with regulations enforced by government.*
the fact that you can walk into ~any restaurant in ~any american city and be highly confident you won't get poisoned is an underrated feature of modernity

Quick tutorial in critical thinking: 1. I read this tweet & thought, "that's odd...never heard of ACP and this seems contrary to what I've previously heard on this topic." 2. Look up the group to learn more about them. 3. Learn that they're listed as a hate group by ADL.
The American College of Pediatricians just put out a 🔥🔥🔥 statement calling out all the major medical associations by name for pushing the gender transition craze on kids. They ask for these groups to "IMMEDIATELY stop the promotion of social affirmation, puberty blockers, cross-sex hormones and surgeries for children and adolescents who experience distress over their biological sex." The full @ACPeds statement ⬇️ "Therefore, given the recent research and the revelations of the harmful approach advocated by WPATH and its followers in the United States, we, the undersigned, call upon the medical professional organizations of the United States, including the American Academy of Pediatrics, the  Endocrine Society, the Pediatric Endocrine Society, American Medical Association, the American Psychological Association, and the American Academy of Child and Adolescent Psychiatry to follow the science and their European professional colleagues and immediately stop the promotion of social affirmation, puberty blockers, cross-sex hormones and surgeries for children and adolescents who experience distress over their biological sex.  Instead, these organizations should recommend comprehensive evaluations and therapies aimed at identifying and addressing underlying psychological co-morbidities and neurodiversity that often predispose to and accompany gender dysphoria. We also encourage the physicians who are members of these professional organizations to contact their leadership and urge them to adhere to the evidence-based research now available.” They link to MANY studies on their site: doctorsprotectingchildren.or…

A few years ago, I had a cool idea: what if I used Amazon wishlists to get homeless people in Vegas what they needed? I could create lists of things they needed and my followers on social media could buy it and it'd get shipped to my house so I could hand it out.

Seems like we have 2 groups of confident, dumb people dominating the comvo in America: 1st group makes brain-dead claims like "nobody is marginalized." 2nd group thinks that spelling "folks" as "folx" somehow helps achieve equity. 2 kinds of clowns, same circus.

When I worked in DC 12 years ago, you couldn't turn around on the Mall without seeing three cops in every direction. USPP, USSS, MPD, FPS, Metro Transit, Smithsonian cops, federal guard forces and special police outside every building , on and on... Where are they these days?
The teens that’s were seen stealing from the CVS In Southeast was also involved in an assault before near the Monument before heading to the CVS.

My Uncle Arnie (Arnold Sachs) is the photographer who took this picture. A random photo that had no significance until it did. I hardly delete any photos.
In 1963, a brief but significant handshake occurred between President John F. Kennedy and a young Bill Clinton, who was then a delegate at the American Legion Boys Nation program. This encounter, lasting mere seconds, would leave a lasting impact on the teenage Clinton, inspiring him to pursue a life of public service and eventually leading to his own presidency. During that summer, two momentous events took place in Washington D.C. that would shape the course of American history. The first was the March on Washington, where Martin Luther King Jr. delivered his iconic "I Have a Dream" speech, which called for jobs and economic freedom. The second was the meeting between President Kennedy and the Boys Nation delegates, including a young Bill Clinton. Clinton, who attended a segregated, all-white school in Arkansas but was a progressive Democrat, was one of the few Southern delegates to vote in favor of a resolution supporting civil rights. A month after the Boys Nation event, King and other leaders met again in Washington for the historic March. Clinton later stated that these two events greatly influenced his early life. When President Kennedy met the Boys Nation delegates, Clinton managed to maneuver his way to the front of the crowd to shake hands with the President. Though their interaction was brief, the video and photographs capturing the moment showed Clinton's admiration and awe at meeting one of his political heroes. The impact of this handshake was evident as soon as it happened. On the bus ride back to the dorms, Clinton excitedly talked about the event and shared his aspirations with his fellow delegates. As future Minnesota Congressman Jim Ramstad recalled, Clinton said to him, "Someday, I'm going to have that job."

I appreciate the rhetorical technique, but both are bad takes. Farming is expensive and good for the entire country, so we should all subsidize it. Higher education is also expensive and also good for the entire country, so we should all subsidize that too.
Why must a college graduate who chose to forgo becoming a farmer be responsible for paying farm subsidies?

I got an NG tube 20 years ago and still vividly remember the pain. Easily in the top 5 worse experiences of my life. If I ever need one again, I'm demanding Versed or even dissociative dose ketamine.
This is not new data, but this video came up in my IG feed, which prompted this. Did you know that NG tube placement is consistently rated as the most painful procedure we do in the ED? It has been ranked worse than abscess I&D, fracture reduction, and Foley placement. This is why I always think twice about necessity before ordering one on a non-intubated patient. If I do order one, it's after a cocktail of viscous lidocaine, nebulized lidocaine, and Afrin. annemergmed.com/article/S019… #emergency #emergencymedicine #foamed #science #pain #patient #research #data #armyemdoc #medtwitter

Even if you didn't know anything about the complexity of EMS, why would you think it wouldn't be expensive? Trucks are notoriously expensive. Payroll is notoriously expensive. Fuel is notoriously expensive. Headlines like this pop up in KY every few years too, like....duh.

No political perspective here; this is just a really good point, medically speaking. Most Paramedics run multiple calls a week for elderly patients who either can't pick themselves up, or whose lives essentially end after a simple fall.
