The body of evidence is abundant that macrophages are key in chronic pain - they are to pain what mast cells are to itch

The Faculty of Medicine honored Dr. Adel Berbari for over six decades of distinguished service to medicine, education, research, and patient care at AUB and beyond.
From the brain to peripheral nerves?
A @NatureComms study shows semaglutide reduces neuroinflammation in mice. Could similar anti-inflammatory effects also help protect against diabetic neuropathy?

I am excited to share my review article, “Amyloid Neuropathies,” published in @ContinuumAAN. My journey caring for patients with amyloid neuropathies began 14 years ago at the Brazilian Amyloidosis Referral Center at my alma mater, @ufrj. Since moving to @MayoClinic, I’ve continued my clinical work and research in amyloidosis, which has given me a unique perspective on patient care across two very different settings. I’ve aimed to simplify this complex topic, including many clinical pearls and a diagnostic flow chart highlighting key red flags.
I’m also proud to share that @LyellJ selected this article as one of the editor’s picks, making it freely available for the next three months. Don’t miss out—download it now! 😊

Lastly, I want to say it's such an honor to write for @ContinuumAAN, which was the most important neurology journal throughout my (long) neurology training journey. The opportunity to give back through this review was very meaningful to me.
I hope you all enjoy it!

Your longest nerves are the first to fail and, it looks like, the last to come back.
A University of Michigan group followed 79 people for two years after weight-loss surgery. On average they lost about 68 pounds.
They took tiny skin biopsies and counted the nerve endings. In the thigh, the count went up. Down in the lower leg it didn't really move.
That fits how these nerves are built. The one that ends in your foot starts near your spine, so it's roughly three feet long, and the cell has to keep all of it supplied. When metabolism goes bad, the far end runs short first. When things improve, it's the last place the repair gets to.
I watched a version of this for twenty years after nerve repairs. Feeling came back in the palm well before the fingertips, and the last stretch was always the slowest part.
So if you've lost weight and your feet still tingle, I wouldn't read that as nothing working. It may just be early.
It's one study, and there was no comparison group. If a randomized trial showed the lower legs recovering on the same timeline as the thighs, I'd drop the "last to come back" part.

Excellent insignts from @DrDSelvarajah Painful DPN is associated with increased mortality, poorer q.o.l but definitely needs better ICD coding in our health care systems.
