The question for your next board meeting isn't "do we have an AI governance policy?"
It's: does our policy describe what's actually operating inside our institution today, or what was operating when we wrote it?
A sophisticated adversary isn't trying to break your security perimeter.
They're looking for the gap between your documented system and your operating system.
That gap isn't protected by your firewall. It accumulated after your policy was written.
The AI your institution approved 18 months ago isn't the AI operating inside it today.
Vendor integrations expanded. Staff adopted tools informally. Features grew beyond the original scope.
The governance policy froze. The system kept moving.
A hotel leaked 1 million passports. Not hacked, just unmapped.
Nobody was watching a system that was running.
That's not a cybersecurity failure. It's a governance failure. And most institutions running AI have the same problem.
