@CapitalOne Your reward point system is a scam. Anytime I want to plan a trip through Capital One Travel your prices for everything are always higher to much higher than other services like Expedia etc… 1/2.

This fucks customers if we want to use our reward points to plan a trip. It significantly devalues our reward points. Also, spending on your CC doesn’t benefit us a whole lot. I have 56k points which only gives me < $600 in $$$. You’re a multi hundred billion $$$ company.

Flights, hotels and car rentals are always higher. This adds no benefit to the customer.

Hospital question… If you get a STEMI patient in the ER, are you starting anti-coagulation, anti-platelet or thrombolytic therapies before you get CT scans to make sure coronary or aortic dissections aren’t present? @smithECGBlog @amalmattu

I think this study is too old but do we know what size BVMs were used in this study? Adult or peds? @smithECGBlog @DitchDoc14 sciencedirect.com/science/ar…

Can we please acknowledge the facts behind using pediatric BVMs for our adult patients? Especially those that are not small females and even more so in patients with SGAs in place. 1/2

In most resp phys texts the avg insp Vt is 500mls +/-. With lung protective ventilation being 6-8ml/kg IBW the peds BVM will only give <300ml max. So 50kg & up @ 6ml/kg will be under ventilated. Add on appropriate 360/90° ext tubing & now we are just dead space ventilating.

@ACEPNow Can you please get with ACS CoT and convince them to had over ATLS to you all and make it more practical, up to date, evidence based or best practice and allow the clinicians (paramedics) who make the most difference in patient care to obtain credentials from the course?

We, yes we, ask this because we have nothing resembling current and competent trauma care. ATLS is the closest to than be needs serious revamping.

Why is TCCC still teaching chest seals on every hole in the chest in the prehospital arena. The true sucking chest would is very much a unicorn and very rare, but covering these holes with dressings and chest tubes usually cause problems. IME it easily makes things worse. 1/2

2/2… If the chest has a hole, leave it uncovered. Yes even your finger thoracostomies. Those 1 way “valved” dressings rarely if ever, work. In the PH realm covering holes and CTs don’t change outcomes. They do cause problems though. @FOAMfrat @SydneyHEMS @fisherad1

Not my Pt… 28 yo M… Syncope after long hike at altitude. Nit witnessed. BP, temp and glucose good. Thoughts? I see SR, HR 80, no blocks, PR depression in some leads. ST ⬆️ in Ant chest leads, ST⬇️ lead III, broad t-waves.

@EcgOxford Maybe, BER due to ST segment shape in chest leads. Maybe right atrial stuff.🤷‍♂️

If you are following silly AHA guidelines & shocking VT w/o a pulse unsyncronized (defibrillation)… YOU ARE WRONG!!! There is no difference in VT with or w/o a pulse. The QRS complexes are still the same and all AEDs are synchronize shocking VT by synchronizing with the R-wave.
