We need to be teaching our students and trainees how to think with machines without letting them think for us.
"We now need to teach them how to interrogate an AI output: how to ask, how to doubt, how to verify, how to recognize hallucination, how to see when a fluent answer has outrun the evidence, and how to preserve the aspects of clinical reasoning that cannot be outsourced."

Visiting professor Brandi Page, MD, from @JHMRadOnc and @HopkinsMedicine, gave a great talk on "Possibilities, Promise and Perspective in Building a Head and Neck Proton Therapy Program" to our residents at Duke #RadOnc/@DukeCancer this morning!

Please join me in welcoming Dr. Kenneth Olivier to the Univ. of Washington as Med Director of VA Rad Onc. He is a leader in thoracic radiation oncology, SBRT, and medical education. We are fortunate to have him join our team!

It is my opinion cancer patients get a type of super power they didn't necessarily ask for. I witness these with admiration on a daily basis.

New in #practicalRO: Tongue-Out Radiation Therapy for Patients With Head and Neck Cancer Facilitated a Rapid Recovery From Post-Radiation Therapy Dysgeusia by Lowering Oral Tongue Dose. tinyurl.com/prowjkil1
Great article for med students navigating specialties. What should we be wearing in #radonc? I’ve seen brachytherapists/head and neck docs (scope day) wear scubs one day a week but some of us rarely wear a white coat to reduce white coat HTN in clinic. What are others opinions?
🚨 La ropa del médico cambia lo que el paciente piensa de ti
No es postureo. Es evidencia.
📊 Revisión sistemática (BMJ Open, 2025): la ropa del médico influye en confianza, profesionalidad y comunicación con el paciente.
🔹 Urgencias → mejor pijama/scrubs
🔹 Consultas → bata blanca genera más confianza

We have become very aggressive in bacterial decolonization to help mitigate oral mucositis in head and neck radiotherapy. It is impossible to say whether both interventions at once are better than either one on their own, but anecdotally I have been pleased. They are essentially harmless interventions to minimize a very difficult and frustrating side effect of treatment.
For those interested, here is a handout we give patients prior to starting treatment:

In our NEURO-ARC study we found that sometimes #radonc residents learn better from a PDF than from a video lecture.
We're now updating all modules to include PDFs, so people can follow along or read it separately.

✨ ASTRO 2025 Reflections ✨
This year’s ASTRO was one of the best I can remember. Enormous credit goes to @SameerKeoleMD for his vision and leadership, it truly came to life in an inspiring way.
One theme stood out above all: the surging interest in functional radiotherapy. The meeting was packed with practice-changing data and insights, plenty for our field to digest and discuss in the months ahead.
To keep the momentum going, I’m excited to announce a 3-Part ASTRO Webinar Series where we’ll explore functional radiotherapy in depth and recap the most important data presented at the meeting.
📅 The first session will be on October 15th:
Pearls and Pitfalls in Non-Malignant Radiation Therapy — Challenging Case Presentations
Featuring:
•Dr. Richard Shaffer
•Dr M. Heinrich Seegenschmiedt
More details to follow next week. I hope you’ll join us for what promises to be an insightful series. #ASTRO2025

A major cheat code in life: Being easy to work with. Listening more than you speak. Being kind when others aren’t. Helping others succeed. Celebrating others’ wins. Showing up early, not just on time. People value those who make their lives easier.

Since I was a #RadiationOncology resident, have always been "struggling" with consistency in target delineation for #postoperativeradiotherapy in #HeadNeckCancer such that I find my volumes "changing" over time!
Kudos to @PierluigiBonomo @sueyom and team at @ESTRO_RT to work on this paper over 2 years and to Mererid for taking it over the finishing line!
Excellent resource for all residents!

Great paper on unilateral vs. bilateral TORS for tonsillar SCC, featured in @QuadShotNews
Contralateral tonsillar primaries: <2%.
Post-op bleeding rate: 7% unilateral vs. 15% bilateral
G-tube rate at last follow-up: 8%
Peri-op MDADI scores: fell from high 80's to mid 70's.
