I won’t lie, I commonly come across poor clinical assessments for low back and leg pain. Distinguishing between Radicular Pain and Radiculopathy is important. When to watch and wait, when to refer, when is more urgent input needed? And, when is it something else? How are you going to decide what to do next without a good assessment? A few handy tips below to differentiate👇 Radicular pain: physical or chemical irritation of a nerve that leads to “Gain of Function”. Typically shocking/burning/electric-like leg pain. Often refers below the knee. Aggravated by neural tension movements! Radiculopathy: physical or chemical irritation of a nerve that leads to “loss of function”. Not always painful! But commonly has pain similar to radicular pain as they co-exist. However, includes a loss of 1 or more of the following: muscle power, reflexes, or sensation (light touch, hot/cold, sharp/blunt). Somatic pain: referred from a structure in the spine eg disc, joint, muscle. Pain can be poorly localised, dull/gnawing, and the back pain is usually worse than the leg symptoms. All neurological tests will be normal. I think there’s room for improvement across the board!!
