HHS just dropped the AI model card requirements for certified EHRs. Clinical AI traceability took a step backward on a problem medicine solved decades ago. Here is what every AI answer should carry, and why anyone liable for patients should demand it.

A clinical AI evaluation should be able to refuse to produce a score. When a held out set overlaps the development set, or the model weights change after sign off, the honest result is no result. Sealed evaluation is what makes the remaining scores worth trusting.

AI scribe use is climbing in Canada, and the concerns being named are errors, consent, and accountability. That ordering matters. Consent before recording, review before signing, and an audit trail after are not features. They are the conditions for using a scribe in care.

Charité, Europe's largest hospital, built its own AI platform instead of buying one, and runs it on its own servers. That is a governance decision: whoever controls the servers, the model versions, and the audit trail controls how AI is used in patient care.

Clinical AI is moving from answering questions to taking actions. The design question changes with it. An agent that can preview, explain, and request review is assistance. An agent that executes without a clinician transfers accountability, whether intended or not.

The next phase of clinical AI is not retrieval. It is action. When a system moves from finding information to acting on it, the requirements change: scoped permissions, a complete audit trail, and a named clinician who can intervene and answer for the result.

Patient-facing AI is now part of the clinical encounter, whether health systems plan for it or not. Patients arrive with model-generated answers in hand. The safety question is no longer access. It is configuration: escalation criteria first, uncertainty stated plainly, and a clinician who remains accountable for the final decision.

AI in medicine will be judged less by benchmark scores and more by whether clinicians can trust it with real patient data. Privacy, reliability, and workflow fit are what matter now. That is what PulseMed builds for hospital systems and clinics.

Where did that answer come from? For every answer our AI Companion gives, we record which model produced it and which sources it drew from.

PulseMed: Physician created AI tools to enhance the physician-patient relationship. AI is everywhere, even healthcare. We believe AI will be used to reduce the administrative burdens in healthcare, allowing physicians to reinvest into what really matters: their patients.
