@roinnslainte & Stephen Donnelly received the Meenan Report in 2020. Rather than read it & implement they decided to produce another report chaired by a defendent in a negligence case & excluding patients/negligence victims. HSE/DOH/NTMA & a director of Merrion Fetal private only

When we started our restaurant 7 years ago. Electric was €1500-1800, bi monthly. Went up then to around €2500. Now it’s €4500. We are locked in now with best deal. Seriously look at costs people before you open anything in this wee country of ours.

The HSE was €580 million over budget by the end of July. At the Public Accounts Committee, I asked a straightforward question: What additional outcomes have patients seen for that extra spending? The HSE outlined the pressures behind the overspend, including rising drug costs, staffing pressures and an ageing population with more complex healthcare needs. These are real challenges. But spending more money does not automatically mean delivering better services. We spend enormous amounts of time debating how much funding our public services should receive. We need to be just as focused on what happens after that money is allocated. What are we getting for it? Are patients seeing better outcomes? Public spending should ultimately be measured by what it delivers, not just how much we spend.

The HSE reports approximately €5.5 billion in procurement spending each year. That's almost a quarter of its overall expenditure. But when I examined its annual reports, I noticed something. The same approximate €5.5 billion figure appeared in both 2024 and 2025. At the Public Accounts Committee, I wanted to establish how that figure was calculated. The HSE confirmed that it was based on estimated contract values, rather than the actual amount spent on procurement during the year. We spend enormous amounts of time debating how much money should go into our public services. But we need to be just as focused on what happens after that money is allocated, how it is spent, what we get in return and whether we are getting value for money. The HSE is one of the largest purchasers in the country. With that comes enormous purchasing power and an opportunity to negotiate better prices, manage suppliers effectively and make public money go further. But to do that properly, you first need good information. And there has been real progress. Following our engagement with the HSE, it reduced its purchase-order publication threshold from €100,000 to €20,000, making significantly more spending visible to the public. The difference is striking: Q1 2025: €305.7m published | 940 records Q1 2026: €694.2m published | 7,918 records That's 127% more spending visible and 742% more individual records published. At PAC, the HSE also confirmed that its new financial management system now gives it much greater visibility of actual purchasing data. Importantly, it committed to providing exact procurement-spend figures for 2025 and 2026 in its 2026 annual report, rather than continuing to rely on estimates. Work is also underway to connect contracts, purchase orders and payments, with a pilot planned for Q3 and reporting outputs targeted for Q4. This brings us closer to what I've been advocating through the Golden Thread: Tender → Contract Award → Contract → Purchase Order → Payment → Outcome It's also why I am bringing forward my Tender to Payment Public Transparency Bill. The State needs to spend better

Power is Power The chart below shows electricity prices in Europe just now, priced per MWh. Just horrific and economically ruinous for any modern economy. The world spends about $4.2 trillion on electricity and this is split between: • Generation (2/3 • Transmission & Distribution (1/3) The world spends around $1.5 trillion on capex and $2.7 trillion on opex. The $1.5 trillion capex gets you 1-2% capacity growth each year. So a rough rule of thumb is that every 1% of GDP you spend on power infrastructure gets you 1-2% more electricity. If you wanted to grow your power supply by eg 8%, you might expect that to cost 6% of GDP. Nobody has the supply chain to do that, if you want to grow this particular supply chain it will cost you more GDP points on second order things. Now… Demand for power around the world is booming, because of 3 big technology trends. 1. Air Conditioning (yes still) +20% consumption 2. Electric Vehicles +80% consumption 3. AI Data Centers +XYZ% consumption Different regions have different progress on these three trends. US has a lot of AC, little EV and a lot of DC, Europe has little AC, lots of EV little DC. China has a lot of AC and EV. Now generation splits into fixed costs for the generator plant (site and technology specific) and variable costs for fuel. Dispatchable Power Cost, by Fuel Type 1. $6 / MWh - Uranium 2. $19 / MWh - Nat Gas 🇺🇸 3. $22 / MWh - Coal 🇺🇸 4. $40 / MWh - Coal 🇨🇳 5. $90 / MWh - LNG 6. $130 / MWh - Biomass 7. $160 / MWh - Nat Gas 🇪🇺 🇯🇵 8. $200 / MWh - Oil Fracking has given the US a structural energy cost advantage. If you talk to people in China about what is the most impressive thing the US has done they say “fracking”. The challenge with US Nat Gas is that it’s now fully developed and expansion is sort of tapped out. What the 21st Century demands is a lot of power, you either have it or you don’t. When you look at planetary scale energy and compute trends you see myriad gigantic arbitrage opportunities. Interesting times!

The Minister for Health @CarrollJennifer is doing her upmost to push ahead with a Commission of Investigation despite the objections of affected families. @SBH_PAG The Farrelly Commission took almost eight years, heard evidence behind closed doors. Its second phase never proceeded, and Grace’s legal representative raised concerns that her voice was not reflected in the final report. We are deeply concerned that this process is being managed to protect the State and its institutions, rather than the children who were so badly failed. And why does the proposed scope exclude scrutiny of @SimonHarrisTD time as Minister for Health? Our children did nothing wrong. They deserve answers in public, not another investigation behind closed doors. We demand a public statutory Tribunal of Inquiry. #BackUs #JusticeForHarvey #PublicTribunalNow

I’ve been saying this for years! Of course in Ireland with our 50% insured population, most of the NHS wannabe civil servants and Slainte-worshipping pols have insurance themselves! The system we should emulate Is Germany.!
Among international experts on healthcare, the UK's system is viewed as being mediocre at best. It is highly centralised, giving patients little or no choice and therefore does not incorporate a competitive/incentivising dynamic. That Ireland's Slaintecare apes the NHS has long been a cause for concern. Sam McBrides's view from Northern Ireland, where the NHS operates

Minister @CarrollJennifer failed to honour commitments made to families on 12 November 2025. Today, she offered no explanation. She appointed Remy Farrell SC, and his terms of reference were published under regulations she herself signed. She cannot now distance herself from the process or its outcome. We have no confidence in the scoping process, proposed inquiry modules or Commission of Investigation. Families will not be railroaded. @SBH_PAG #BackUs #JusticeForHarvey #PublicInquiryNow

Today, the Dáil will hear Statements on Children’s Health Ireland The published @roinnslainte minutes of our meeting with Tánaiste Simon Harris and Minister for Health Jennifer Carroll MacNeill on 12 November 2025 clearly record the commitments made to families. sbhpag.com/uploads/1/4/0/1/1… What actually happened? 🚫 No Tribunal of Inquiry. The Minister did not bring the promised proposal for a Tribunal of Inquiry in principle to Cabinet. Instead, a Commission of Investigation has been recommended. 🚫 No agreed mediator. The Minister appointed a facilitator without agreement or collaboration with parents and advocacy organisations, despite the commitment to an agreed mediator. 🚫 No independent clinical expert. Despite discussions about appointing an independent clinician to assist the process, no such appointment was made. 🚫 No direct consultation with children and young people. The scoping report identifies no children or young adults living with scoliosis or spina bifida who were directly consulted. 🚫 No further meaningful engagement with families. Families requested further engagement before the terms of reference were brought before the Dáil. That engagement did not take place. Families came to the table in good faith. We are not asking for new promises. We are asking for the commitments made on 12 November to be honoured. @SimonHarrisTD, you were an active participant in that meeting and supported the commitments made to families. Simon Harris, you made a promise to our children in 2017 that was never delivered. You made commitments again in November 2025. Do not fail our children twice. Honour your word. #BackUs #JusticeForHarvey #TribunalNow @SBH_PAG

Today, myself and @Stephen63224725 collect the last of Harvey’s ashes. From there, we head in to the Dáil to listen to the statements on the inquiry in to Children’s Health Ireland. @CarrollJennifer @SimonHarrisTD you both know what was agreed to, and you both know the current recommendations mean this inquiry won’t be fit for purpose if you push on with it as is. #JusticeForHarvey #TribunalOfInquiry #BackUs
