🧵 Tweetorial: 1/ MRI is now central to prostate cancer workup — but does what we see on MRI predict outcomes? In a national cohort of 2,275 veterans, we found that MRI-detected EPE is common & highly prognostic.
MRI-detected extraprostatic extension, including major capsule abutment, is common in localized #ProstateCancer (PC) and independently prognostic. MRI findings should be considered in the next iteration of PC staging. acsjournals.onlinelibrary.wi… @OncoAlert #RadOnc #PCSM

🚨#ESMO26 Localised stage II-IIIB gastroesophageal cancer; n=1024 patient trial from 🇨🇳 indicates de-escalated 4 months of adjuvant XELOX is non-inferior to 6 months XELOX . V important as tolerance of these regimens is a big issue! #gcsm @marklewismd

🚨🔔🔔Very happy to announce that we have randomised the first patient into @TOGAANZ and @TROGfightcancer phase III, PRIME-lung trial! First site open @PeterMacCC ➡️ this newly open 420 patient phase III trial investigates whether the addition of high dose radiation to the lung primary in patients with non-small cell lung cancer can be life-prolonging (primary endpoint OS) ➡️ Thank you all investigators and patients, this is just the beginning! #lungcancer #lcsm

1/6: I am excited to share our multi-institutional F-SHARP phase I/II trial, now published in @JCO_ASCO! doi.org/10.1200/JCO-25-02730 There are limited prospective data defining the optimal management of intraprostatic radiorecurrence (IPR) after definitive RT for prostate cancer. We asked: 1. Is dose-escalated focal salvage high dose rate brachytherapy safe and effective for IPR? 2. Can Decipher (@Veracyte) identify patients with higher risk of recurrence with salvage ReRT that may benefit from therapeutic intensification? 🧵👇

In other words, post-treatment PSA does not (completely) overrule pre-treatment risk. Same for undetectable PSA after RP. And probably true for pCR/MRD/RCB after neoadjuvant treatment.

An important topic that is frequently discussed during many GI tumor boards. How do we conceptualize/treat oligoprogressive and OMD? Thank you @KrishanJethwa for mentoring and working on this with me! apm.amegroups.org/article/vi…

Make that THREE papers this week! 🚨 RAPTOR is out: 3,120 men & 11 RCTs. After RT alone, being BCR-free at 5y marked a functional-cure benchmark (8y DM 6.6%; 10y PCSM 2.5%). W/ ADT, late metastatic risk persisted. doi.org/10.1016/j.euo.2026.0… @AmarUKishan @DrSpratticus @alison_tree

I discuss this with all my UIR patients and it certainly has been a factor in some of them choosing surgery. It is part of the treatment package. Fortunately we have great investigators evaluating who may be a candidate for deescalation.

Yes you raise an important point @DongNguyeb - short term ADT is often part of the package deal of radiotherapy for unfavorable intermediate risk prostate cancer. And of course causes sexual problems. Good news is there are emerging biomarkers (ARTERA and now Decipher) that are helping us select which pts to omit ADT and do radiation as a Monotherapy. Approximately 2/3 can omit! I’m an optimist. There are exciting, better days ahead for our patients with prostate cancer for each treatment modality. My main point I will continue to share is patients should understand ALL standard options— and consult with both urology and radiation oncology—and should decide what’s best for them. I think we both agree on this 🤝🏾
Fine. And RadioOnco should also note this: the advantage of SBRT over RP in sexual function was only assessed under the conditions of the PACE-A study, where ADT was not permitted. This does not reflect current practice, because after SBRT, omitting STADT in unfavorable intermediate-risk patients is still not permitted , yet this group was part of the PACE-A inclusion criteria. Meanwhile, for RP, guidelines do not recommend adjuvant ADT. So, the favorable result of SBRT over RP in sexual function only reflects the confines of the study where ADT was not allowed for the enrolled patients and does not reflect the reality that SBRT still lacks sufficient evidence to omit ADT.
