🦴 Tibial Fulcrum Test Evaluating localized shin pain? The tibial fulcrum test can help assess for a tibial bone stress injury. With the patient supine, stabilize the lower leg and apply a controlled force perpendicular to the tibia, opposite the painful area, creating a bending stress across the bone. ✅ Positive test: Reproduction of the patient’s localized tibial pain. Interpret findings alongside the history, examination, and imaging—a positive test alone does not confirm a stress fracture. 🔗 Link: wikism.org/Tibial_Fulcrum_Te… #TibialFulcrumTest #TibialStressFracture #SportsMedicine #PhysicalExam #MedEd

🏀 A 14-year-old basketball player presents with gradually worsening pain just below the right kneecap over the past month. The pain increases with jumping, running, and kneeling and improves with rest. There is no history of acute injury. Examination reveals localized swelling and tenderness over the tibial tubercle, with pain during resisted knee extension. Knee range of motion is preserved, with no joint effusion or instability. Findings are consistent with Osgood-Schlatter disease. What would you recommend for management? 🩺 Courvoisier Sign: A Clue to Biliary Obstruction A palpable, enlarged, nontender gallbladder in a patient with painless jaundice raises concern for malignant biliary obstruction, including pancreatic head cancer or distal cholangiocarcinoma. 🔎 On examination, a smooth right upper quadrant mass may be felt moving with inspiration. 💡 Clinical pearl: This finding suggests malignancy but is not diagnostic—benign causes of obstruction can also produce it. 🔗 Link: wikism.org/Courvoisier_Sign #CourvoisierSign #ClinicalExamination #MedicalEducation #PancreaticCancer #BiliaryObstruction

👃 A 24-year-old basketball player presents after taking an elbow to the nose two hours ago. He reports nasal pain, swelling, and a brief nosebleed that resolved with direct pressure. Examination reveals nasal bridge tenderness and a new visible deformity. Both nasal passages are patent, with no septal hematoma, open wound, malocclusion, or visual symptoms. The X-ray shows a displaced nasal bone fracture. ❓ What is the most appropriate management? 👍 The Thumb UCL Stress Test evaluates ligament stability at the thumb MCP joint following a suspected skier’s thumb or gamekeeper’s thumb injury. 🖐️ Stabilize the first metacarpal and gently apply valgus stress to the proximal phalanx. Assess in full extension and approximately 30° of flexion, comparing with the opposite thumb. ⚠️ Pain may suggest injury, while excessive laxity or an absent firm endpoint raises concern for a complete UCL tear. 🔗 Link: wikism.org/Thumb_UCL_Stress_… #ThumbUCL #SkiersThumb #GamekeepersThumb #SportsMedicine #PhysicalExam

👍 The Thumb CMC Grind Test assesses for osteoarthritis at the first carpometacarpal joint—the joint between the first metacarpal and trapezium at the base of the thumb. 🖐️ Technique: Stabilize the wrist, grasp the first metacarpal, and apply gentle axial compression while rotating the metacarpal. ✅ Positive finding: Reproduction of the patient’s familiar pain at the thumb base, sometimes accompanied by grinding or crepitus. 💡 Clinical pearl: A negative grind test does not exclude thumb CMC osteoarthritis. Interpret the finding alongside the history and other examination findings. 🔗 Link in bio. #ThumbPain #CMCArthritis #GrindTest #PhysicalExam #SportsMedicine

👍 Thumb MCP Extension Test helps assess for thumb CMC arthritis. ✋ Stabilize the patient’s hand and place a finger on the dorsal aspect of the proximal thumb. 🦶 The Thompson Test, also called the calf squeeze test, helps identify an Achilles tendon rupture. With the patient lying prone and the feet extending beyond the examination table, squeeze the calf while observing the foot. An intact Achilles tendon produces plantarflexion. Absent plantarflexion is a positive test, suggesting a complete rupture. 💬 What sport do you most commonly associate with Achilles tendon ruptures? With the patient seated, place your thumb on the dorsal surface and index finger on the plantar surface of the symptomatic intermetatarsal space.
