As a palliative care Dr and academic I know that palliative care cannot solve all suffering.
But “forced drowning, slowly over 10 hours in your own faecal vomit”?
This does not happen. Its barely physiologically possible. And would NEVER happen with even decent palliative care.

It is personal not party; those who’ve seen loved ones die in excruciating pain despite wonderful palliative care, feel that surely we should be able to do things better?
