IMAGINE ... an oral investigational first-in-class lambda light chain stabilizer for the treatment of AL amyloidosis. HUGE. Developed by Dr. Jeff Kelly and colleagues who developed the research behind tafamidis. The clinical trial called SHIELD-01 is about to begin and will be showing up on clinicaltrials.gov soon.

Thanks so much for talking with me about what we have accomplished and what is still to be done to #cure #amyloidosis @DanaFarber
Enjoyed sitting down with Giada Bianchi to discuss what the all-cause mortality benefit seen with anselamimab - which targets circulating amyloidogenic light chains rather than the plasma cell clone - tells us about what's really driving AL amyloidosis mortality.

Observation in amyloidosis: Daratumumab can kill active plasma cells. Question: Can it help in pre-🫀 transplant desensitization? Answer: Yes! Excited to publish this study (a true labor of love!) in @JCardFail. onlinejcf.com/article/S1071-…
Bravo @MatthewHoMD !!
When is thrombosis merely coincidental with an M-protein, and in which thrombotic disorders is there evidence that the M-protein itself can be pathogenic?
In our new review doi.org/10.1002/ajh.70467, we propose a framework for Monoclonal Gammopathy of Thrombotic Significance.

.@DanaFarber investigators have developed a platform for systematically discovering molecular glues that could become protein degradation drug candidates. The research described today in @Nature builds on previous @fischerlab1 and Ebert Lab findings that revealed the potential for molecular glue degraders to benefit patients with cancer. Very happy to have @CuiminB at JHU! This is partly because of my own interest in models of bounded rationality, an area in which she excels. More broadly, it’s because she’s a top theorist who strengthens an already outstanding theory group. – at Chevy Chase Village, MD

Systemic amyloidosis: From “untreatable” disease to a new era of targeted therapies
For decades, systemic amyloidoses were considered rare, devastating diseases with limited treatment options and often fatal outcomes. Today, advances in molecular biology and immunotherapy have transformed amyloidosis into a highly treatable group of disorders, highlighting how understanding disease mechanisms can directly translate into life-changing therapies.
Amyloidosis occurs when misfolded proteins aggregate and deposit in tissues, causing progressive organ dysfunction. Two major forms dominate clinical practice:
🔹 Transthyretin (ATTR) amyloidosis
🔹 Light chain (AL) amyloidosis
For ATTR amyloidosis, decades of research into transthyretin misfolding and aggregation led to the development of therapies targeting different stages of disease biology. Since 2018, six novel therapies have been approved, including approaches that stabilize transthyretin or reduce production of the pathogenic protein.
In AL amyloidosis, treatment has been transformed by plasma cell–directed therapy. The combination of:
💊 Daratumumab + cyclophosphamide + bortezomib + dexamethasone (Dara-CyBorD)
became a major advance after FDA approval in 2021, improving hematologic responses and organ outcomes.
The field is now entering another phase with the emergence of immune-based therapies, including:
🧬 Bispecific T-cell engagers (BiTEs)
🧬 Other next-generation immunotherapies targeting the pathogenic plasma cell clone
Early clinical trials in relapsed AL amyloidosis have shown remarkable activity, raising the possibility that deeper and more durable disease control may be achievable.
However, successful treatment depends on early recognition. The key clinical message is:
Think amyloidosis early → confirm the diagnosis quickly → accurately type the amyloid protein → start disease-directed therapy without delay.
This shift represents one of the most remarkable stories in modern hematology: a group of diseases once considered rare, mysterious, and almost uniformly fatal are now becoming examples of precision medicine in action.
Future challenges remain — particularly achieving true cures for AL and ATTR amyloidosis — but advances in protein biology, targeted therapies, and immune-based approaches have fundamentally changed the outlook for patients.

Many thanks to the @BIS_org for inviting me to the 25th BIS Annual Conference. I discussed an excellent paper by @R2Rsquared on the inflationary consequences of the Iran shock. My main take: The Iran shock signals the risk of a more fragmented world going forward
Congrats @bloodgenes and co-recipients. This is so well deserved. Thanks for bringing this all the way through from bench to patients! Amazing.
🙏 Deeply honored to receive the 2026 Warren Alpert Prize! This award reflects the work by so many to enable transformative therapies for sickle cell disease and thalassemia, and the power of human genetics to reveal biology and guide new treatments!
