ICU Studies - Return to the Classics: It’s been ~25 yrs since the ARMA study showed that ventilation of ARDS patients w a lower tidal volume (Vt) results in ⬇️ mortality & ⬆️ the number of days without ventilator use. It’s also common belief that using volume-controlled (VC)

This is quite esoteric but I’m glad someone else is also nerding out on it! There is another approach to estimating pulm capillary pressure described by Teboul et al They did so by wedging the PA catheter distally balloon deflated. Figure explains why👇 xitter.cf/thind888/status/135385…
ICU Physiology Secrets - Return to Basics Edition: If you are placing Swan-Ganz (SG) catheters or you like reading/interpreting their waveforms, this is for you: You walk in a patient’s room exactly when your fellow intensivist tries to “wedge” a newly placed SG catheter:

ICU Hemodynamic Secrets: Did you ever manage a patient where you were convinced that there was a patent foramen ovale (eg in a case of cryptogenic shock or unexplained hypoxemia) but the bubble study did not reveal a R-to-L shunt? I came across this study 👇

ICU Physiology Secrets (for nerds): An important effect of LV dysfunction (LVD) is pulmonary edema due to ⬆️ in pulmonary venous pressure (PVP), presumably as a direct hemodynamic consequence of LVD. This was a computer-based analysis of cardiovascular hemodynamics:

Time for answers! 🤓 1) This parasternal long-axis cardiac view shows significant RV dilation (though we can’t see the full RV in this view, we can certainly appreciate that it is not normal). This is confirmed in subsequent views; along with reduced RV function. We also see an anechoic space posterior to the LV, which tracks anterior to the descending thoracic aorta (DTA). This is a pericardial effusion.
It's almost Hallowe'en 🎃, so time for some terrifying #POCUS images 👻🧙‍♀️😱 1) What pathology do you see in these views? 2) What clinical scenario might have caused this? 3) How would you manage this patient if they were stable? If they were crashing? (More clips below)
