C. Marasmus.
The term monkey face (simian appearance) is classically associated with severe marasmus due to marked loss of subcutaneous fat and muscle, giving the face an aged, wrinkled appearance.
The first thing that comes to my mind is severe respiratory distress with increased work of breathing. 🚨
The prominent neck muscle activity and possible retractions make me think of accessory muscle recruitment, suggesting that the patient may be struggling to breathe.
My immediate concerns would be:
* Acute respiratory distress
* Severe airway obstruction or bronchospasm
* COPD or severe asthma exacerbation
* Possible respiratory failure
The priority: Assess airway, breathing, respiratory rate, oxygen saturation and overall clinical condition immediately.
One question comes to mind: Is the patient getting enough oxygen, or is respiratory fatigue setting in? 🩺
The video alone cannot establish the diagnosis.
This shows a lingual mucosal horn (sometimes referred to as a papilloma or a mucosal horn of the tongue).
What it is: It is a rare, benign overgrowth of the oral mucosa on the tongue, typically presenting as a small, firm, finger-like projection or horn made of keratinized tissue.
Characteristics: It usually develops at the expense of the papillae on the tongue. While it looks unusual, these lesions are generally harmless and can be easily removed by a doctor or dentist if needed.
The medical abnormality shown in the image is Diprosopus (also known as craniofacial duplication).
What it is: It is an extremely rare congenital condition where all or part of the facial features are duplicated on a single head and body.
Cause: Unlike true conjoined twins, diprosopus is not caused by the incomplete separation of two embryos; rather, it results from abnormal signaling activity of a protein during embryonic development (specifically the Sonic Hedgehog or SHH pathway).
