This is in part why I’ve been more than happy, honored, and humbled to work with @Wiki_Guidelines and @BradSpellberg to further these processes into real time adjustments, given the speed of medical knowledge nowadays. There’s no excuse for not having an ongoing robust discussion that is updated for bedside clinicians to be able to tackle the versioning medical crisis bequeath to us by our “elders”
One of the recurring concerns with the IDSA AMR Guidance is the heavy reliance on in vitro susceptibility data, PK/PD principles, and animal models, while often giving less weight to higher levels of clinical evidence, including randomized controlled trials and well-conducted observational studies. Preclinical data are invaluable for hypothesis generation, but when robust clinical outcome data are available, they should take precedence. Guideline development also requires intellectual humility—recognizing uncertainty, acknowledging the limitations of the available evidence, and being willing to revise recommendations when higher-quality clinical data emerge. That is how evidence-based medicine advances and how patient care improves. @BradSpellberg @DrToddLee #idxposts

I don't think the alternative is "who knows." Nor do I think it's arrogant to acknowledge uncertainty. The alternative is intellectual humility. When robust clinical evidence is lacking, it's appropriate to acknowledge uncertainty rather than express a preference based largely on animal models. That's a core principle of the @Wiki_Guidelines : when the evidence doesn't support a clear recommendation, we present the pros and cons as a clinical review and provide clinicians with reasonable options rather than a preferred choice. Our @wikiguidelines letter to the editor regarding the controversy between IDSA and ATS on CAP guidelines is now published. Time for societies to modernize their approach to guidelines to STOP creating care standards based on opinions. @ABsteward @DrToddLee @AnilMakam @medrants

It’s fascinating how a single RCT can shake decades of ‘standard care.’ That’s the beauty (and challenge) of evidence-based medicine. Medical guidelines must embrace the humility of uncertainty! @Wiki_Guidelines @BradSpellberg #IDXposts
Time to rethink SBP prophylaxis? Antibiotic prophylaxis for SBP in cirrhosis has been standard since the 1990s—but the new 🔥ASEPTIC trial, presented at #TLM25, just challenged that. 📊 Bactrim vs placebo in cirrhosis + ascites (no prior SBP): ➡️ No survival benefit for primary prophylaxis Maybe it’s time to revisit this long-standing practice!

Common 🆔 occurs commonly, which means they occur in immunocompromised patients as well. The @Wiki_Guidelines initiative spearheaded by @BradSpellberg addresses osteo, endocarditis, and UTIs. Apparently cranberry for UTI prevention is recommended‼️ transplantid.net/?guidelines…

Im not concerned with strength of recommendation. In the @Wiki_Guidelines approach, there is no strength to recommendation. It's like Yoda, either do, or do not. I AM concerned with strength of evidence. The evidence for short course CAP Tx is overwhelming. It is NOT weak.
