We're heading to @athenahealth Thrive in Nashville, Tennessee, November 2-4. VP of Network Development, Kristina Contrera, alongside Anthony Spano, executive director of value-based care at athenahealth, will be sharing where #ValueBasedCare is headed and how our partnership helps practices turn strategy into everyday workflows. Maureen Padden, MD, will moderate a conversation with Aledade partner physician Umar Bowers, MD, of Dawson Med Primary and Urgent Care on how independent practices improve patient care and workflow efficiency.
Come find us at booth 901 and let's talk about what's next for #PrimaryCare.

Clinicians do their best work when they can stay focused on the patient in front of them, with everything they need already at hand. That's what our electronic health record (EHR) workflow integration is built to deliver: bringing relevant, patient-specific insights directly into the chart a physician already has open, so relevant gaps in care can be identified easily.

More than 30 Aledaders joined the broader accountable care community at the Fall 2026 @NAACOSnews conference in Washington, D.C., to collaborate on efforts to advance value-based care nationwide. At the event, SVP of Policy and Economics @Travis_Broome joined a panel on optimizing network design for accessibility and value, while VP of Population Health Impact Tamara Thomas chaired a session on leveraging the Rural Health Transformation Program to advance accountable care.
Our ongoing work with NAACOS remains vital to empowering #PrimaryCare organizations with the solutions they need to thrive in #ValueBasedCare.

ACO vs MCO: The acronyms sound similar, but the two models operate differently.
An accountable care organization (ACO) is led by clinicians who coordinate a patient's care, work to improve outcomes and take responsibility for the total cost of care. A managed care organization (MCO) is led by a health plan that works to manage costs through its provider network and coverage rules. For primary care organizations evaluating value-based contracts, understanding where the responsibility for care decisions rests is the best place to start.

Thank you to @lightspeedvp for the engaging conversation with @Farzad_MD.
Healthcare’s last technology revolution didn’t prevent patient strokes. New incentives might help.
@Farzad_MD has been working to improve health outcomes like reducing the stroke rate for much of his career in medicine and public health.
He believed that converting to electronic health records (EHR) would help, but after transitioning physicians over, the numbers didn’t change.
Farzad co-founded @aledadeaco to financially reward higher value care.
He joined Partners Dr. @BrentonFargnoli and Dr. @GalymImanbayev on Venture Doctors to discuss the company he’s building and his mission to help independent primary care physicians thrive.
Farzad dives into how Aledade is leveraging AI, why he thinks founders in their forties are underestimated, and why he believes the real differentiator in healthcare AI is trust rather than capability.

When #PrimaryCare teams can't see what's happening across a patient's full care journey, from specialist visits to hospital discharges, they can't coordinate care effectively.
In our latest piece with @BeckersHR, we explore how health systems can close those visibility gaps, ease administrative burdens and support stronger care coordination, all without rebuilding their existing infrastructure.

We are proud of Aledade's participation in the National Committee for Quality Assurance (@NCQA) Health Innovation Summit, where our leaders and partners presented best practices for improving blood pressure control. VP of Population Health Impact Tamara Thomas moderated a session with panelists MaryCatherine Arbour, MD, MPH, VP of clinical quality, Martine Molett, RN, CCM, MHA, executive director of population health and value-based care at @ChristHospital and Keila Brown, DNP, chief medical officer of Central Mississippi Health Services.
Together they shared how care teams are improving blood pressure control, even in communities facing real barriers like limited transportation, food access and broadband. Their shared message: care teams can save lives when they adapt proven approaches to their practice and the people they serve.
Pictured (from left) are Tamara, Martine, Keila, MaryCatherine and Maya Ray, senior director of performance improvement.
Aledade partner physician @EricRHansenDO joined David Mandell on the Wealth Strategies for Today's Physician podcast to talk about independent practice and #ValueBasedCare. He shared with his fellow doctors, "You're doing the work already. You should be getting compensated for that."
That's the heart of Dr. Hansen's message: physicians delivering high-quality patient care deserve financial stability, and participating in an accountable care organization (ACO) is one way to make that happen.

This National #PrimaryCare Week, we're celebrating the physicians who show up for their communities every single day. Our clinician partners don't just treat patients, they treat neighbors, friends and family, bringing a level of care that only comes from truly knowing the people you serve. That kind of connection is exactly what #ValueBasedCare protects, giving physicians the freedom to practice medicine the way they've always imagined.

Thanks to @AledadeACO for having me on The ACO Show to talk about California, our partnership, and my thoughts about AI! Listen here: podcasts.apple.com/us/podcas…
In this episode of The ACO Show, David Ford (@theehrnerd), VP of Health Information Technology at @CMAdocs and CEO of @cma_pso, joins hosts Joshua Israel, MD, and @dc_cavanaugh to talk about #ValueBasedCare in California.
Ford says the real differentiator for practices succeeding in value-based care isn't size or resources — it's culture. "It's really about in the practice, is everyone willing to change and are they all rowing in the same direction?"
He also sees artificial intelligence as a way to help organizations like his scale the hands-on coaching practices needed to make that shift — as long as physicians help shape how it's built.

The 30 days following a hospital discharge mark one of the most critical windows in a patient's care journey. Transitional care management (TCM) bridges this gap by requiring interactive outreach within two business days and a clinical visit within two weeks — right when readmission risk is at its highest. The impact on patient outcomes is clear: structured follow-up directly lowers avoidable hospital readmissions, reduces post-discharge mortality and catches medication errors before they escalate.

Too many people get caught in a broken healthcare system. We believe there's a better way: #ValueBasedCare. For more than a decade, Aledade has partnered with #PrimaryCare organizations across the country to help physicians remain independent, financially strong and clinically excellent, so patients get the proactive, preventive care they deserve. When primary care is strong, everyone benefits, physicians and patients alike.

In this new edition of the Aledade Digest, we highlight our formal comments to @CMSGov on the proposed 2027 Physician Fee Schedule and how Hill Day carried that advocacy to Capitol Hill. We'll also revisit how the right technology is helping #PrimaryCare teams simplify their workflows, and the latest episode of The ACO Show, where @NCQA President and CEO @vgargMD makes the case for a simpler path forward on quality measurement.

.@ES_RuralHealth (ESRHS) in Virginia has provided a #PrimaryCare safety net for its community for 50 years, depending on their limited staff to cover everything from conducting wellness visits to helping patients navigate everyday challenges. Through their partnership with Aledade, ESRHS' care team was able to identify patients with unmet care needs and streamline outreach for annual wellness visits, cutting down on administrative burden so staff could focus on #PatientCare.
The results speak for themselves, said Health Education Coordinator Paige Annis, BSN, RN, CCHW: "Last year we completed over 2,000 annual wellness visits. We more than doubled our numbers in a year and a half. We love Aledade."

A Medicare Annual Wellness Visit (AWV) offers primary care teams an important opportunity to catch health risks early and transform long-term patient outcomes. By combining health risk assessments, preventive screenings and medication reviews, AWVs provide the foundation so clinicians can build personalized, proactive care plans for their patients. Research suggests* prioritizing these structured annual visits helps catch problems before they become crises, lowering acute hospital care costs while keeping patients healthier and more engaged.
