Bless me Father, It has been 10 years since my last great saphenous vein stripping. I have done 5000 lasers. Today I did a strip because I had to. It went well. It will be another decade I am sure.

Vascular ligation with silk suture 🩸🪡 One of the most essential lifesaving surgical skills in the operating room. Identify the vessel ➝ carefully isolate it ➝ pass the silk ➝ secure the knot with proper tension to achieve hemostasis. Precision matters, because one small mistake can mean uncontrolled bleeding. ⚠️ Simple skill… but it carries huge responsibility in surgery. 👨‍⚕️🔬 #Surgery #VascularLigation

Power is not duty!!! What seems easier, turns into a real tragedy after 5-10 years. The most worrying thing is the change in medical training that these devices have left behind. Several generations without proper training are left open. The bill will be much more expensive.
🚨 16 years after TEVAR's approval in Japan (2009), the bill is coming due. Our center's data (2003–2025, n=8,690): Stent-graft explantations are surging. → 32–35 cases/year since 2022. Post-approval trend: +2.3/year (p<0.001) 🔴 Paraplegia: 4.1% vs 1.0% — 4× higher (p=0.003) 🔴 In-hospital mortality: 6.4% vs 3.6% — 1.8× 🔴 OR time: 386 vs 307 min (p<0.001) Late TEVAR complications start emerging at ~5 years. A wave of salvage surgery hits at ~15 years. TEVAR is not a panacea. Patient selection is everything. #AorticSurgery #TEVAR #EVAR #Aorta

Much needed device. The question still remains. Do we need to remove suprarenal fixation component? Or just cut and leave them.

When the urologist remove a renal tumor and postoperatively, the legs swell… the cava… When GYN puts a Trocar through the iliac vein… When Gen surgery puts a trocar through the iliac artery… When the trocsr car goes to the aorta… When the Critical Care physician sees Frank blood coming out of the chest tube… When the postop Dialysis counter placed in the neck – portable x-ray reveals it is in the carotid artery… When the upper arm swells up like a tomato from my Radio Art heart Cath… On the groin smells up like a grapefruit after E.
