ALT Histological examination revealed a tumor composed of cells with irregular, sometimes grooved nuclei, eosinophilic cytoplasm, and frequent mitoses (black circles), and with scattered pleomorphic multinucleated or binucleated cells (arrows) (A). Hemophagocytic activity was present in some cells (B). A brown cytoplasmic or extracellular pigment was present (C), which corresponded to hemosiderin deposits as demonstrated by Perl's immunostaining (D). Neoplastic cells were positive for CD68 (E), CD163 (F), and focally positive for CD4 (G). CD3 immunostaining demonstrated an abundant infiltrate of small lymphocytes (H).

ALT Histopathological features of the lesion. (A), (B) Histomorphology revealed two distinct components. (A) Showing a highly cellular area with spindle-shaped tumor cells and interspersed ganglioid cells. (B) Showing a sparse cells area with inconspicuous tumor cells, and the presence of astrocytes and neurons cells. (C), (D) The tumor cells are negative for INI1 staining. Star symbol (*) indicates infiltrative tumor cells in the hypocellular areas. (E), (F) The Ki-67 proliferation index in the hypercellular (E) and hypocellular (F) areas. Magnification ×400 (A)–(D), ×200 (E), (F).

ALT Comprehensive molecular characterization of a cohort of 126 pathologically confirmed meningeal solitary fibrous tumors, including DNA methylation profiling, revealed the presence of three distinct methylation clusters: Cluster 1 (n = 38), Cluster 2 (n = 22), and Cluster 3 (n = 20). These methylation clusters were significantly associated with NAB2::STAT6 fusion breakpoints (fusion type), TERT promoter mutation status, histologic phenotype, and metastasis-free survival.
