โIgnorance more frequently begets confidence than does knowledge.โ
I think itโs time to rethink medical school and accelerate the path to becoming a doctor, since the job has absolutely nothing to do with just maintaining book knowledge.
We could probably shrink medical school length in half.
And a good portion of that time should be how to leverage AI to diagnose things

Expanding ED space to cope with crowding is analogous to installing a much larger toilet bowl to cope with a blocked sewer outlet.

We don't need new, gameable, metrics for corridor care to understand this apocalypse. The evidence on long waits is already good. The extra focus on corridor care is a distraction from paying attention to long wait evidence...

Long waitsโwhether in corridors or notโare a major cause of higher mortality (maybe more than 20k extra deaths per year). see rcem.ac.uk/press-release/howโฆ ...

When the obsession with targets misses the point
Steve Blackโs analysis exposes a classic management failure: focusing on a visible symptom while losing sight of the problem causing it.
Corridor care is a symptom. Long waits are the problem โ whether patients are in a corridor, a waiting room or a cubicle.
Focus on corridor-care targets and you risk masking the true scale of the crisis, distorting priorities and diverting precious management time into collecting more data instead of fixing the causes.
Deming warned against managing by targets instead of improving systems.
โPower to the Middleโ argues for empowering managers to solve problems, not burying them in bureaucracy.
And this isnโt new. The lessons were there before 2010. The NHS already has data on long waits.
Sixteen years later, we donโt need another metric to count the symptoms. We need leaders to confront the cause, use the evidence and empower people to fix it.

This becomes more obvious when hospitals are asked to collect new data on the ages of corridor care cases. This will take time and waste effort which is not needed...

I wonder if they have appropriately increased their staffing in anticipation of winter pressures, or if they have participated in a hiring freeze that increases pressure and burnout for current staff?
Nobody seems to be investing in staff upfront these days.

If you cut what youโre offering to pay NHS staff to do extra work, theyโre more likely not to do the extra work you need to get done.

Begs the question of "why?" cos GP is a bargain. ยฃ140 per list patient per annum for median 5 consultations. 380 m appointments a year for 7% of NHS budget (compared to say 30 m A&E attendances or outpatient attendances)
I think they want more direct control and less challenge

Iโll keep saying it. The problem isnt the GPs and their work ethic or receptionists. The problem is demand. That we cannot meet for what we get to look after a patient - less than a netflix sub

Edwards-Deming warned about targets:
When you manage people by numbers, they learn to hit the numbers. Can get very good at it, find creative ways to hit those targets.
But not necessarily to improve the system.
Targets can encourage gaming, hide problems and shift attention away from quality.
The danger? The dashboard looks better while the system gets worse.
We should be improving the system rather than managing the numbers.

They can, however, help 'game' the target and, whilst the target is set at 78%, means it is now possible to 'meet 'the target and create the illusion of improvement, whilst leaving the most vulnerable patients (to whom the target matters) languishing in the dept for hours/ days

Where should Doctors discharge patients who are medically fit for discharge but need care in community as they are not back to their pre admission level of independence/function @YvetteCooperMP ?

Worse, current policy and a good deal of external advice has completely missed the underlying problem and there are no plans at all to fix it.

Also the obsession with "neighbourhood health" in the 10 year "Plan" (a misnomer) shows a government that is hostile to traditional General Practice (and District Nursing/Healthg Visiting which has been decimated) . They would rather fund anything *but* GPs and DNs

We have wards full of people who are medically fit for discharge, but awaiting social care.
So they push us to discharge those who are still medically unfit because that is โin our giftโ.
