Last week, the UAB Cardiovascular Institute (@UABCVI) was among the first in the world to complete the first valve-in-valve procedure using a new transcatheter aortic valve replacement system.

We have had a great initial experience with Farapulse and Ensite X! The mapping system coupled with ICE in the left atrium ensures proper catheter positioning for therapy delivery. @AbbottCardio @Segarspr @BSCCardiology @WilliamMaddoxMD

Highlights from my talk on OAC in device detected atrial fibrillation (DDAF):
- DDAF affects 30% of pts w/ CIED >65 yo
- Stroke risk related to longest duration and CHA2DS2-VASc
- DDAF <5 min not clinically significant but >24h approaches clinical AF if elevated C2V
- >6 min DDAF ⬆️stroke risk 2x but absolute risk is low ~1% (even with C2V ~4)
- OAC in DDAF ⬇️stroke but ⬆️major bleeding (treating 1000 pts will prevent 3 strokes and cause 7 bleeds)
Better tools needed to determine which pts will benefit most from OAC
