Paediatric solid tumours: when is ancillary testing essential? 🔬
In a retrospective study of 309 children at an expert paediatric oncology centre, Bruinsma and colleagues found that morphology alone was sufficient for diagnosis in 58.9% of cases, with IHC needed in another 15.2%. Molecular testing was necessary for diagnosis in 25.9%, particularly for soft tissue tumours and undifferentiated small round cell sarcomas.
These findings concern establishing a diagnosis. Molecular tests may still be needed for prognosis, risk stratification and treatment, and results from an expert centre may not apply equally elsewhere.

AI can flag prostate cancer. But is FDA clearance enough?
This Editor’s Choice review explores FDA-cleared AI tools for prostate cancer.
In FDA multi-reader studies, AI assistance improved pathologist sensitivity:
Paige: 89.5% → 96.8%
Ibex: 90.5% → 93.9%
But improved sensitivity comes with important considerations, including specificity, reader experience and real-world validation.

Are our pathology guidelines keeping pace with reality?
📖 A thought-provoking proposal to make diagnostic criteria practical, accessible and globally applicable in this excellent paper by the authors - Philippe Benjamin Stephenson, Olaleke Folaranmi, José Jiménez Heffernan, and Sanjay Mukhopadhyay, published in the BMJ Journal of Clinical Pathology.
Three powerful messages:
🔹 Make criteria feasible across resource settings
🔹 Focus on what truly impacts diagnosis and treatment
🔹 Guideline panels should include community pathologists and represent the global community of pathologists
The best guidelines should not just define what is ideal—they should help every pathologist make the best possible diagnosis with the resources available.

🧬 ALK positive. But is it truly actionable?
In high-grade pulmonary neuroendocrine carcinomas, ALK IHC positivity may not tell the whole story.
Among 8 ALK IHC-positive cases in this study, only 2 had a confirmed ALK rearrangement, and both were detected by RNA sequencing.
🔬 IHC positive ≠ ALK rearranged
🧬 RNA sequencing identified fusions missed by DNA testing and even FISH in one case
🎯 Durable benefit from ALK-targeted therapy was seen in the molecularly confirmed DCTN1-ALK LCNEC case, with ongoing benefit beyond 16 months.
Takeaway: Don’t treat the stain. Confirm the driver.

Can Ki-67 counting go digital—without a whole-slide scanner? 🔬💻
A new JCP study puts several digital approaches to the test for Ki-67 quantification in gastrointestinal well-differentiated neuroendocrine tumours (NETs).
- 68 NETs were evaluated using manual counting vs digital image analysis
- QuPath, ImageJ and ChatGPT-based colour segmentation showed strong agreement with manual Ki-67 counting
- Even simple microscope camera-captured images performed comparably to whole-slide images
- Grade changes were uncommon—and occurred mainly around the critical Ki-67 cut-off values
- Some platforms performed better than others, highlighting the importance of validation before clinical use
Why does this matter?
Digital Ki-67 assessment may not have to depend on expensive whole-slide imaging infrastructure. Open-source tools + a microscope camera could make more reproducible quantification accessible to a much wider range of laboratories.
