I was always intrigued by Prof Leng not going through with having a defined SoP. Instead this has shifted to trying to define 'clinical protocols' with 84 stakeholders. Leave it to expert organisations to define. Enforce it via the GMC. Then have timely reviews 🤷‍♂️
Response to @RCPhysicians ask re @lengreview clinical protocol done- as a Fellow of RCP. I believe in transparency- so here are my views on some of the key questions. In summary? Needs a rejection on key areas of PAs seeing undifferentiated patients. And certainly not lean on Gen Next to be liability sponge. And let @rcgp make their own decisions regards primary care- no need to interfere. NB: Don't like it, give a counter view. Don't abuse or be snarky. It's a democracy- it's my view. You are welcome to give yours. @mmamas1973 @djnicholl @Mumtaz_Patel_1 @OGMustafa @BDChadwick Link: nhssugardoc.blogspot.com/202…

FWIW, the standardisation of training does not involve the diagnosis of conditions in undifferentiated patients.

Important for @RCPhysicians members and fellows to share their views on the PA proposals now that these are in the public domain Useful to reference the @lengreview and draft RCP PA scope Interested to hear from resident and LE doctors as well as consultants rcp.ac.uk/news-and-media/new…

Question @BucksHealthcare How long is it acceptable to leave an 86yr old Nil by mouth without a review? Surgeons at Stoke Mandeville Hospital might be busy but…? 24 hours ❌ 36 hours ❌ 48 hours 🤷🏻‍♂️ Longer? Asking for a Mum! GP & I know it’s already been too long!

Why can't the @AoMRC take a lead to do this? They are heavily subsidised by fees paid for my the RCs, who are in turn funded by it's members.
There is no *UK* consensus (largely due to GMC failing to take a lead). WHO and other countries have established guidance. One has to wonder why GMC and RCs struggle so much.

Is there a way to disable video reels on this site and YouTube? 🤔

Still rotating every three months (FTE) across various hospitals 😔
"Rotational training is an obvious target for reform. The answer is not to abolish rotations—different hospitals provide experiences that others cannot, but doctors should rotate when there is an educational need, not as an end in itself." @DrAsifQasim bmj.com/content/394/bmj-2026…

ECPR for refractory OHCA reaches >35% survival in well-organized systems — and far lower elsewhere. That gap is largely system design. Which means it's something we can fix. New contemporary review in @JAHA_AHA on how to build one. 🧵

Leanne QN: "Hi, I'm Leanne a Queen Nurse. I would like to propose a new treatment. It's well-researched and evidence-based." Patient: "Sure, can I see the evidence please" Leanne QN: "No". Block. Is this not a probity issue?

For clarity, the above is a hypothetical example to demonstrate the absurdity of my recent exchange over the last few days with Leanne.
