Post Trial Survey and Patient Advocacy for Continued Access to Simufilam
Something isn't making sense with the $sava trial results. If you or a family member were in the trial and you would like to help advocate to get access to the drug, please fill out this form. You can help us make certain the data represents the ACTUAL trial results.
Caregivers, clinical site investigators, all reported the drug was working but the data did not represent what was said.
The safety profile of the drug is incredible and many families and medical professionals at the site believe the drug works. Just because it is at a small biotech does not mean it should not have a legitimate chance to help with #alzheimers
Anything you can do at clinical trial sites to register patients will help. If you are an advocate for the drug, call the sites and suggest they send patients to this link with this QR code. If you want to help, maybe make a printable image they can hand out. Let's get as many of the 2200 patients registered as possible.
I am not sure if we will be able to drive change, gain access, but if we don't try we never will know. We will use your collective voice to advocate to Cassava and the FDA.
I do not recommend buying the stock in support of this effort. This is about the patients and the families.

The simufilam Phase 3 trials for #Alzheimers may have patients in earlier in the AD progression than expected - let me explain...
Cassava Sciences, $sava, allowed patients that are 16 to 27 MMSE into the trial. 16 to 20 are considered Moderate and 21- 27 are considered Mild.
To understand the points I am about to make you need to understand the analysis that $lly performed on their donanemab Trailblazer ALZ 2 trial. They accepted patients with MMSE between 20 and 28, but performed amyloid AND tau PET scans to confirm Alzheimer's disease in the brain.
Plasma pTau181 was used for screening in the Phase 3 Cassava trials. Plasma pTau181 detects AD progression earlier that PET scans. In other words there could be patients that Trailblazer PET scan eliminated because they were earlier in disease, but pTau181 would catch.
Something else that Trailblazer taught us was MMSE is not nearly as good at predicting disease state as PET pTau or Plasma pTau tests. The higher tau cohort performed significantly worse at 18 months with a 11.08 adas-cog13 decline vs 4.9 in the low and medium. For a 2 point difference in average MMSE, according to the ITO study of ADAS-Cog declines, you would only expect a 1 point different decline at 18 months.
MMSE really should not be used alone to determine disease state. Trials should really use plasma pTau tests to screen. pTau is a much better predictor of true disease progression than MMSE.
All of this has two impacts to the phase 3 trial of Cassava:
1. If we believe milds do better on simufilam, then the pTau181 scan will produces a milder patient base than one that is confirmed with PET.
2. The 16 to 27 MMSE allowance may still produce a more moderate base of patients, but when balanced with the earlier disease screening of pTau181, I think the overall population of RETHINK will decline ADAS-Cog12 at a slower rate than I originally thought. After this analysis, I think the ADAS-Cog12 placebo decline will be 4.25 or less. The trailblazer placebo for their total population declined at 4.45 at 12 months.
The ADCS-ADL is the second primary endpoint for Cassava. Based on what patient families say, I feel comfortable about them out performing placebo. The issue I am finding is that there is not much available research for performance. One that I found found a population with MMSE of 21 (Refocus is 21.7) declined ADCS-ADL by 6.7 points [1].
So I am forecasting RETHINK placebo for change in ADAS-Cog12 of 4.45 or lower and change in ADCS-ADL of 6.7 or less. I believe the treatment arms will be near zero for both. A conservative estimate is they cut both declines in half, so 2.25 and 3.34. Those are my bets... what do you think they placebo and treatment performance will be?

$SAVA @elonmusk @RobertKennedyJr @VivekGRamaswamy FINALLY GOT MY “BLUE CHECKMARK (took 3 days BTW) because I like what you and your team are doing. Let’s see how this goes as I try to bring attention to Cassava Sciences (appended link below) and the CORRUPT WALLSTREET FORCES against Alzheimer’s innovation…
With the 4 year long Short and Distort against Cassava Sciences (and countless other companies) by CORRUPT WALLSTREET, and with wallstreet’s use of the weight of our government as well as our Courts, the SEC and DOJ, and the use of paid “FUD consultants” and paid legacy media propaganda (NYT, WSJ, Science, STAT), along with their failure to delivers, naked shorting, options manipulation, millions of shares borrowed each day, all in order to try to stop a company that’s is attempting to develop a safe drug for the hardest to address healthcare unmet indication on the planet. This completely proves to me that our degenerate US stock market is on par with second/third world stock market corruption, which makes our US stock market a joke under current SEC.
There are only two sell-side analysts (with zero buy side analysts) covering $SAVA weeks out from Phase 3 and very rare positive media coverage, all consistent with this vast Wallstreet COLLUSION and they have driven this FALSE narrative against Cassava. Wallstreet can’t bear the thought of buying the majority of share supply from thousands of retail investors (who have out smarted them) and would rather steal, lie, and deceive and lawfare in order to destroy $SAVA rather than buy back their shorted shares and payback their illicit debt…it’s as simple as that!
I have summarized the historic discovery Cassava Sciences has made (@joerogan @bryan_johnson @LoganPaul #Longevity #FLNA) appended post below-MUST READ, and many other posts therein) and we are only weeks out from Phase 3 Alzheimer’s Rethink study data readout! It’s not hyperbole to state that no other biotech or pharmaceutical company has discovered and developed a therapeutic targeting the biological fate of cellular Filamin A (FLNA), and Cassava Sciences is the first to do it, This is a FACT! This will impact many diseases that are in mild-moderate stages, not only Alzheimer’s!
Wallstreet has tried to stop Cassava’s Phase 3 Alzheimer’s program and have failed. The implications of targeting FLNA with a potent small molecule therapeutic will shock the world and Wallstreet’s corruption will be further laid bare for everyone to see.
$SAVA @elonmusk @RobertKennedyJr There is NO STOPPING SCIENCE no matter how hard CORRUPT WALLSTREET (and we all know who these disgusting criminal Hedgefunds are) try to stop Cassava Sciences by using the recent weight of the government by the bogus SEC actions against $SAVA and the bogus DOJ action against one of the co-inventors, Dr. Wang…. teapaw.org/Kalonshaka/status/1851…
I have posted about this criminal short and distort here.. teapaw.org/Kalonshaka/status/1827… and here.. teapaw.org/Kalonshaka/status/1828… and here.. teapaw.org/Kalonshaka/status/1839…
After Cassava Sciences reads out their phase 3 rethink study by EOY, I have very little doubts that it will replicate all the data summarized here…teapaw.org/Kalonshaka/status/1827… and here…teapaw.org/Kalonshaka/status/1841…
And Cassava has recently said that they are also pursuing additional biological studies with Simufilam as further support (though this is not really needed after phase 3 successful readout imo) AND they are planning and gearing up production of Simufilam to address the gargantuan Alzheimers market!
This continued collection of data will further open up the research flood gates into the FLNA pathways and FLNA implications is no many diseases and Cassava has the only drug therapeutic approach to address this pathogenic FLNA pathway. I have posted about this on numerous occasions and different topics and my speculated models and summaries are all based on existing science that no one has articulated before. I summarized these here:
1. The supporting evidence of different (altered) FLNA confirmations here based on FLNA phosphorylation… teapaw.org/Kalonshaka/status/1826…
2. The extremely potent pico-molar/femto-molar biological activity (confirmed by outside researchers) of simufilam in comparison to most other drug-protein interactions…teapaw.org/Kalonshaka/status/1826…
3. The connection of FLNA to atherosclerosis.. teapaw.org/Kalonshaka/status/1832…

Corrupt Wallstreet that started this short and distort against Cassava over 3 years ago were completely oblivious to all this and even Cassava to this day is playing down what they have discovered IMHO! I’ll repeat, the WORLD changes after Phase 3 readout and there’s NOTHING STOPPING SCIENCE!

$SAVA …..
ADAM AND HIS CREW 🤡
$SAVA Nemesis, Adam Feuerstein, Contemplates Success in Simufilam for Mild #Alzheimers Patients
Adam knows that simufilam does not have drug related side effects like the approved $lly and $biib MAB treatments for Alzheimer's. These trials were performed on early and mild patients, so the whole population was significantly more mild than the simufilam trial on mild to moderate. A success on just mild for a safe pill for #Alzheimers would be a blockbuster success for simufilam. He goes from calling the drug inert and the study futile to considering success in mild.
Alzheimer's is an awful disease and I am certain the FDA will work with $sava to get approval if they show statistically significant improvement in the patients in the mild cohort. Rick Barry said on the call that if they showed promise on mild but not moderate that they could revise the Statistical analysis plan for Refocus to add more Alpha to the mild cohort. He is setting the narrative to be negative on a positive readout for mild patients in phase 3.
Adam goes on to say that the FDA will not approve just mild. He says that the whole population must perform for both ADL and ADAS-cog12, but he fails to mention Rick's comments about changing the mild ALPHA on REFOCUS. REFOCUS is an 18 month study with over 1000 patients. Time is more important in AD studies, so greater and more statistically significant results are more likely in that study.
He did not discuss the data presented by Lilly in detail, but $LLY removed ~550 patients from their primary population with high tau by PET from their ~1700 patient study. Also, they stacked their ADL and ADAS Cog in their special population to claim a 35%. When the whole population was included they only reduced decline of ADAS-cog by 19%. Additionally, APOE4 carriers performed dramatically worse and they make up 60% of the AD population. Their study was strange too because they halted dosing the drug if the patient showed clearance of plaque. The patients in the 18 month study were not even on the drug the whole time. Patients on the drug need constant monitoring of inconvenient PET scans. It requires visits for IV administration. Unfortunately, all of the MAB drugs are a total trainwreck. But hey, they pay Adam's salary, so he is going to be hesitant to talk negative about them.
The truth is that compared to what is on the market, simufilam would be a blockbuster success if it only shows success in Mild. I have seen patient success in Mild and Moderate. I think REFOCUS and RETHINK will show statistically significant results for all the patients, so this discussion will be unnecessary in the end.
AD3 𝕏 retweeted
RETHINK phase 3 reads out by Cassava Sciences, $sava, around December 18th of 2024. We know the drug is safe, so it comes down to effectiveness that could be demonstrated in the RETHINK readout.
Here are my price estimates depending on data:
1. RETHINK does not show stat sig improvement in mild or moderate - Price will likely goto $10 or lower and wait for REFOCUS, but not good and shorts win. 5% chance this happens
2. RETHINK shows stat sig for Mild and not for moderate and not for total. Lets say Mild slows decline by 30% or similar to $lly and $Biib MAB treatments. Stock will goto $50+ after readout then drift up toward a $10B market cap or $250. 5% chance this happens
3. RETHINK beats the MABS and slows mild patients cognition 50% better than placebo. Moderate not significant and total not significant. The stock should goto $80+ then drift to $350 because it will be a blockbuster. 5% chance this happens.
4. RETHINK reads out MILD as improvement in cognition, so a 100% or greater than placebo. Moderate not significant, but overall is significant. Stock should goto $200 instantly, then drift up to $500+. Proves that if you take simufilam early you will not get AD symptoms. 20% chance this happens
5. RESULT I EXPECT - RETHINK reads out as Mild improvement AND Moderate better than placebo. So Mild's improve over 100% to placebo and moderate improve 30-50% decline than placebo. Stock will instantly goto $300 plus and drift toward $2000. Mega blockbuster drug, but slower uptake. 35% chance this happens
6. RETHINK on average improves the entire treatment group cognition at 1 year, so improvement over placebo > 100%. Stock will instantly goto 500 and rise toward $3000+. Mega blockbuster drug that solves the AD crisis and world wide medical and government will get behind getting the drug out as fast as possible. 30% chance this happens.
There are more scenarios based on the 2 different primary endpoints. It's hard not to seem crazy when you put out potential values 10-100x the current stock price. Look at the TAM in the image though... Also, Rick Barry just said on the conference call that the actual patient population is DOUBLE because he says that Primary Care Physicians only diagnose 50% of AD patients. They are doing commercialization planning right now. The TAM could be double this...
There are so many variables at this point on pricing, diagnosis technology and etc. I think the treatment effects, ease of administration, and safety profile will quickly become known to ALL families affected by #Alzheimers around the world. The small molecule pill is EASY to scale. I think that the patient and trial site personal interviews are not ONE OFF, but PERVASIVE in the trial. This stock is a raging buy at $26.

The Consistency of the Positive Data from $sava Points to Phase 3 Success
All this remarkable trial data reported here was reviewed by the DOJ and SEC and not flagged with any issues
Phase 3 data coming ~December 15.
TLDR: What are the CLINICAL results showing us? How are the patients doing on the drug? What are the people closest to the results saying? What sticks out to me is a quote from Remi Barbier “It's the consistency of data from the basic research to the animal data, to the CSF biomarkers, to the early cognition, to the open-label study. There's a consistent theme that, in my opinion, the world of Alzheimer's disease R&D has never seen before.” Every import data point I have reviewed points to the drug working.
All of these show consistency of data aligning to make phase III very very likely to be positive (details below):
1. 5 - Phase 2 and Phase 3 trial site investigators speak of patients doing better on the drug
2. 8 Patients Stories of Dramatic #Alzheimers symptom reversals or stabilization with mild to severe disease states. Dropouts within historical norms despite nasty short seller attack and press. hmm?
3. Cassava Team and Board consistently VERY excited about the prospects of the drug. Talented professionals stuck through all of the investigations and short seller attacks
4. Suzanne Hendrix, a very experienced Alzheimer's biostatistician, spoke of open label cognition FAR outside historical placebo.
5. NONE OF THIS DATA WAS CALLED INTO QUESTION BY THE SEC OR DOJ. ALL OF THIS FALLS OUTSIDE OF THEIR CLAIMS. WE KNOW THEY REVIEWED EVERYTHING, SO IT ADDS MUCH MORE CREDENCE TO THE ACCURACY OF THE REPORTING OF THEIR OPEN LABEL STUDY RESULTS
1. Clinical Trial Sites Positive about the Drug -
The team has extensive conversations with the clinical trial site investigators. James Kupiec, Chief Medical Officer, said "We have expanded the open-label trials in direct response to requests from our clinical research sites". I think he is hearing this from the sites, but he doesn't tell us. They cannot ask trial sites how the patients are doing, but the trial sites can tell them. When patients we know call their trial sites, this is what they say about their experience:
“You should have heard the excitement in her voice! She said it is going to be so nice to finally have a safe option that helps people improve versus the MABS that don't really work and cause brain bleeds. “
The open label patient families I have spoken to talked directly to the trial sites as they were in the trial. The clinical trial sites are overwhelmingly positive about the drug. They are all very experienced with #alzheimers trials and some of them experienced tragic deaths caused by the MABs at their trial sites.
Two phase 2 trial site investigators spoke out about the drug.
Dr Gonzalex-Rojas ran Phase 1 & Phase 2 clinical trials for Simufilam for Cassava Sciences as an ACRP certified Principal Investigator. She treats hundreds of Alzheimer’s patients annually and ran 13 different Alzheimer’s drug trials.
"After month two and 3, several of the patients that we have on the open labels, the husband, the wife of the son, the daughter, they're telling me that some patients are improving. For example, one of the patients that didn't remember much the names and the past important person or phone numbers and now he's showing better it's recall in these these activities.
I have another patient of Cassava also that told me that the patient was get confused it took a shower or not and sometimes used to have like a wet underwear in the in the shower to make sure that she didn't take a shower again that day and the wife is telling me that this is not happening anymore that he kind of is
able to tell that they if a patient took a shower or not"

Dr. Boris Nikolv - Research Physician and CEO of IMIC
I have spoken to Boris a number of times. The short attack had a dramatic impact on their business, BUT the emerged very very successful and are doing great now. The FDA audited their site due to the short seller attack and passed with no actions required. When asked about patients on the study he said:
“We see a lot of patients through these studies. We have seen improvements in many of the patients. Including descriptions of the patient by the caregiver. For example a patient caregiver that came to us the other week and told us that the patient started cooking again she used to burn every time that she cooked on the stove and now she can cook again small simple meals but she was not able to do that before and there is a lot of cases like that… a lot.”

Phase 2 OLE - extended again
Aug 2022 - July 2024 Still going strong. No changes
Patient 7
14 MMSE still stable after 10 months on treatment in REFOCUS OLE - Historical placebo would decline 2.5 MMSE or 5 ADAS-cog11 over similar timeframe
She was in $sava simufilam Phase 3 Refocus -
Diagnosed 2016 with Alzheimer's
Blinded Refocus 18 Months
July 2022 - Jan 2024 : 17-18 MMSE at start declined to 14
Refocus 12 Month OLE
Jan 2024 - July 2024: On the medicine for 6 months maintained at 14 after dropping 1.3 MMSE points every six months while blinded . Husband believes she was on placebo, but stabilized. He believes there is a treatment effect on the open label.
UPDATE Oct 2024
"She is doing well. She is still showing no signs of agitation and I believe is stable. We are at the clinic next Tuesday. Thanks for asking, generally very optimistic…."
For reverence a 14 MMSE normally would decline about 5 ADAS-cog11 or 2.5 MMSE points over this period. Untreated over a 10 month period you would see disease progression on key AD symptoms. These estimates are from the ITO study of placebo populations.
This patient is the first I have seen evidence of stabilization of a low Moderate AD (I called her severe last update but a 14 is not severe, but it is below the entry criteria for the drug) patient from simufilam. It aligns with my theory that some of the Phase 2 readout volatility was the later Phase 2 trial sites screened poorly and allowed patients with co-pathologies. Hence, the milds are more representative of treatment effects from simufilam because they are much less likely to have co-pathologies. Particularly the 87 that made it all the way through the 24 mark on the OLE.
Patient 8
I do not have much detailed information on, but from a credible source they said there is another remarkable patient story from a clinical trial site. I am not 100% sure that this patient does not overlap with the other 7 or from a site that is different from the other 5. I have found that stories I hear through the grapevine have overlap. The first 7, I directly interacted with their families and know their real identity.
I have had no patients that reached out to me saying the drug is dangerous or that they are not doing well. The drug clearly did not work on some patients based on the clinical data... just haven't heard from anyone in that state.
AAIC Individual Patient Scatter Plot 2021 August
A chart of the 50 patients at 9 months from AAIC in August of 2021 sheds light that the story of the patients above align with the broader patients in the open label. The patient stories that I have heard seem to point to some moderate patients are the super responders.
This chart aligns with that theory as it would be very surprising to see a mild improvement to a perfect score. It is more likely that moderates are the ones that improved 7-14 points on ADAS-Cog11 at 9 months. The average ADAS-Cog11 baseline score for mild's was 15 and the moderates were 25.7. Again, this was reviewed by the SEC and not mentioned by their reports. To me, this validates a full detailed review of this data by the SEC showed it was reported accurately.

1. Rick Barry said "suffice to say, we are believers" in the drug.
2. Remi Barbier "It's the consistency of data from the basic research to the animal data, to the CSF biomarkers, to the early cognition, to the open-label study. There's a consistent theme that, in my opinion,
the world of Alzheimer's disease R&D has never seen before. "
Being Patient Interview with Remi
youtu.be/z8ipamyLuuQ?si=lrnU…
3. Suzanne Hendrix - She is one of the foremost experts in Alzheimer’s disease endpoint analysis. She has EXTENSIVE experience and knowledge of what is normal for placebo’s in AD.
She analyzed the open label results against other AD Placebo’s with patients with similar baseline demographics with Mild-to-Moderate. She said
"The treatment effect with simufilam from the open label study so essentially what this is showing is that the pattern of decline that was seen in the mild to moderate treatment arm was a lot less decline than you would normally see with a placebo group of similar populations so whichever placebo group you used there was quite a bit more decline than was seen in that active arm in the open label”
This is the chart that she is referring to:
Then they analyzed just the mild patients. Alzheimer’s studies now are mostly done on early to mild patients. This patient group is where simufilam really clearly separates from the studies with similar baseline characteristics. She said
“The active simufilam group [confidence interval] barely overlaps with the mildest of the placebo arms from those mild and early AD studies”
Note, the one it overlapped with was EARLY AD, so there was no overlap with the mild studies of about 15,000 mild placebo patients
She goes on to say
“Patients with mild AD, simufilam stabled ADAS-Cog11 scores scores over 18 months and based on the comparison to the historic data you can tell that that's pretty unusual to have that much stabilization over that long of a time period”
Finally, here is her analyzing the 18 month data with mild patients. She says:
“Again, the decline that we’re saying is substantially less than you would normally expect based on comparison to these historical control arms.”
This 18 month data is incredibly powerful because the longer you go, the placebo research says, it is more likely that the data is showing separation from the typical placebo. And as you can see, for the mild patients, the 18 month data further separates from the historical placebo’s. These studies are EARLY AD compared to mild. Early AD declines slower than Mild AD.

Finally, the 24 month readout, No Decline in Cognition Scores in Patients with Mild Alzheimer's Disease Who Received Simufilam Continuously For 24 Months. This is even further outside historical norms. This was 87 patients for two years. The open label study was performed in the US at 17 different US sites that are experienced with ADAS-Cog evaluations and Alzheimer’s studies. The screening procedure was similar to other AD screening that these sites were used to performing.

(@MartinShkreli,
These comments from Suzanne were relevant to the comments you made about placebo’s. There are two purposes to placebo’s… 1) To make it so the patient does not know whether treatment is administered, so they are less likely to “invent” symptom remediation. In AD this stops showing in 1 year. See research below. 2) To align patient populations to similar baseline characteristics. In AD, historically placebo performance has wildly varied and given false hope to early trials. Great performing placebos have also caused drugs with promise to fail. It is actually more statistically reliable to compare an AD trial to a large number of AD placebos than to run one yourself. It does require you to do enough patients for at least 12 months to compare to other studies. Here are two references to historical placebo analysis that I use to understand typical placebo performance:

I unblocked you because of the short sellers you seem to come with some good faith to the discussion. Lots of off the wall comments with no evidence, but some recognition of some facts or low likelihood events like trial pauses. FYI, patients I talked to are hyper aware of all the bad press Cassava gets and are frustrated and fearful the drug will be taken from them. They say the SEC, DOJ and press are violently mischaracterizing the situation and they are benefiting from the drug. Adam and the others are not protecting patients. They are harming them.
