🧠 Late-Onset Unprovoked Seizures and Subsequent Dementia Risk Our new study in Seizure: European Journal of Epilepsy @SeizureJournal examines the association between late-onset unprovoked seizures of unknown etiology and subsequent dementia-related outcomes in older adults. Using federated data from the TriNetX Global Collaborative Network within a target trial emulation framework, we analyzed 80,802 propensity score–matched individuals. Late-onset unprovoked seizures were associated with higher subsequent risks of all-cause dementia, Alzheimer disease, vascular dementia, and mild cognitive impairment, with the strongest association observed for vascular dementia (HR 2.07). These findings describe an observational association, not causality, and raise a broader question: whether late-life unprovoked seizures may represent an early clinical marker of underlying neurodegenerative or cerebrovascular disease. Further longitudinal studies integrating seizure phenotyping, biomarkers, and neuroimaging will be needed to clarify this relationship. 🧠 The Circadian Clock in Glioblastoma: Mechanisms and Therapeutic Potential Our new review in Clocks & Sleep examines the emerging relationship between circadian biology and glioblastoma, with particular attention to molecular mechanisms and potential therapeutic implications. Circadian rhythms regulate a broad range of physiological processes, and increasing evidence suggests that disruption of the molecular clock may also influence glioblastoma initiation and progression. In this review, we discuss alterations in core circadian genes, their potential involvement in glioma biology, and whether restoring circadian regulation could contribute to tumor control. We also explore chronotherapy as a potential therapeutic strategy. Aligning chemotherapy and radiotherapy with individual biological rhythms may offer a way to optimize treatment response while accounting for temporal variation in tumor and host physiology. Current evidence remains largely mechanistic and exploratory, and further experimental and clinical studies are needed before circadian-based strategies can be incorporated into routine glioblastoma care. 🧠 New paper in Neurological Sciences with Diogo Haddad Santos: Amyloid-beta-targeting monoclonal antibodies in early Alzheimer’s disease: a Cochrane review summary and appraisal for the neurological community Building on the recent Cochrane review, we examine how class-level estimates should be interpreted alongside agent-specific evidence on efficacy, ARIA risk, target engagement, and clinical relevance. A central message is that pooled class effects should not be assumed to represent individual antibodies such as lecanemab or donanemab. We acknowledge the Cochrane Neurological Sciences Field @SpringerNature, the editorial team of Cochrane Review CD016297, @cochranecollab, and @cochranecollab for the evidence synthesis that provided the foundation for this critical appraisal. 📄 link.springer.com/article/10… 📖 cochranelibrary.com/cdsr/doi… #AlzheimersDisease #Neurology #Dementia #Cochrane

🧠 Epilepsy and Subsequent Dementia in Adults with Down Syndrome Our new study in Seizure: European Journal of Epilepsy @SeizureJournal examines the temporal relationship between epilepsy and subsequent dementia in adults with Down syndrome. Using federated data from the TriNetX Global Collaborative Network, we found that epilepsy was associated with approximately twice the subsequent rate of all-cause dementia and Alzheimer disease after propensity score matching. The association remained consistent across age and sex strata and under a more restrictive definition of epilepsy. These findings establish a temporal, not causal, association and raise an important question: whether epilepsy may represent part of the clinical trajectory of Alzheimer-related neurodegeneration in Down syndrome. Further longitudinal studies integrating clinical phenotyping, biomarkers, and neuroimaging will be needed to address this question.
