Thank you for this @tomperriello Let’s work to get more docs to rural areas and keep rural hospitals open. AI is great for helping physicians but cannot replace physicians
AI cannot replace a human doctor, no matter how much the insurance lobbyists want it to. We need to stop handing out blank checks to corporations who only have malicious intent for our rural communities.

Our healthcare heroes are the canaries in the coal mine. They are alerting us of a impending collapse– and we need to listen.

BREAKING: @ACPIMPhysicians joined patients, pediatricians, & other physician groups today in suing the Administration over its Medicaid work requirements rule, which unlawfully strips protections for medically frail patients. Millions of vulnerable Americans could lose coverage.
A new rule would unlawfully introduce barriers, including a work requirement, for people on Medicaid who are currently exempt. We’re challenging this rule to protect health coverage for thousands and ensure they don't miss appointments, lose access to meds, or experience worsening health symptoms.

Not an easy advocacy ask, but more and more important as docs are overwhelmingly employed by large systems
I’m excited to share this commentary just published online. Rethinking Advocacy for Physician Payment - The American Journal of Medicine amjmed.com/article/S0002-934…

CMS seeking comment about how others view this process and about alternatives to the CPT book/process that could be utilized. Hmmm. On that note, will sign off for now.

Finally, CMS has a lot to say about the CPT book. This has been a big area of concern of the HHS secretary. CMS is seeking comment regarding conflicts of interest inherent in the process & about the monopoly that AMA has over CPT. It’s a shot across the bow of @AmerMedicalAssn

Medicare next has a LOT of questions about how technology will change primary care, how doctors are using AI, and how AI may change things like the annual wellness visit. I appreciate that CMS is forward thinking about this critical area.

This to me seems more intuitive than creating different categories of E/M codes, some for use by primary care doctors & others for use by consultants. In other words, prospective payments are a better way to recognize the chronic, longitudinal work of primary care clinicians
30
Personally, I have been increasingly excited about a formula for primary care that combines E/M fee for service + successful participation in an ACO (the quality piece) + some prospective payment (to recognize the longitudinal care part) such as the APCM monthly payments.

34
CMS touches on formalizing prospective primary care payments and/or creating new E/M codes that are used specifically for doctors who are managing patients longitudinally as opposed to episodically.

I appreciate the importance CMS is placing on primary care as it tries to reimagine how to support it as the backbone of a highly functioning healthcare system, and in the setting of rapid advances in technology

Moving along, Medicare is requesting input on how to redesign primary care for modern times. CMS is looking for creativity and states “ we are increasingly concerned about the relative under valuate of primary care.“
33
Historically, CMS accepts 80% to 95% of RUC recommended values for services. This year, they were at the low end of that range, accepting 80% of RUC recommended values of services. (Table A-D9 p 216)

Decreasing reimbursement for the injection by half doesn’t seem fair. CMS should justify why a 50% reduction is necessary. CMS can certainly do a better job clarifying the exact nature of any overlap and tailor reductions and payments accordingly.

As just one example, imagine the number of times I see a patient for management of their diabetes, hypertension, a fib, & hyperlipidemia, where they ask “oh by the way Doc, can you do a Cortisone shot on my knee today?“.
