MASTERPLAN full publication is out!! The RT volumes are worth a closer look. CTV = primary GTV + nodal GTV + tumour-vessel interface. The TVI extends 5 mm proximally and 5 mm distally along involved/closely adjacent vessels, for patients where surgery is likely. PTV = CTV/ITV + 5 mm. No elective nodal irradiation, and no elective perineural or celiac/SMA plexus volume specified in the protocol. That is a focused target, with gross nodal disease included. Real-time RTQA is a big part of this story. Review identified 36 major planning deviations in 12 patients, reduced to 2 deviations in 2 patients after QA. All plans passed independent review before treatment. As a radiation oncologist, I want that detail alongside the outcomes. Reproducing the treatment means reproducing the contour review, OAR constraints and image guidance too. Also worth keeping the treatment delivery denominator in view: only 40/49 patients assigned to SBRT actually received it (82%). The efficacy analysis is intention-to-treat. It should not be read as an outcome series of patients who all completed SBRT. The surgical numbers need some unpacking. All comparisons below are chemo + SBRT vs chemo alone: ✅ Underwent surgery: 62% vs 44% (29/47 vs 11/25). ✅ Completed resection: 53% vs 44% (25/47 vs 11/25). ✅ R0 among all resected patients: 52% vs 36% (13/25 vs 4/11). ✅ R0 in the resected POPC subset: 62% vs 40% (13/21 vs 4/10). POPC is the potentially operable cohort. So the 62% surgery figure includes exploration without resection, while the 62% R0 figure applies only to resected POPC patients. These are different endpoints with different denominators. None of those between-arm surgical comparisons was statistically significant. The paper also defines R0 as more than 1 mm clearance. Adjuvant chemo is another important difference: 80% vs 36% of resected patients received it (20/25 vs 4/11). That complicates attribution within the treatment pathway. Early progression in the control group may have contributed, so this is a post-randomisation imbalance with more than one possible explanation. The post-hoc analyses were directionally consistent, but the authors explicitly caution about selection bias. For the planning details: PTV_EVAL excluded the GI PRVs, with a minimum 3 mm PRV expansion. OAR constraints took priority over target coverage. Duodenum, stomach and small bowel D0.5cc were each constrained to <33 Gy, with V30 <5 cc. Delivery used standard linacs; breath hold was recommended, and fiducials were placed in 35/40 patients (88%). This is relevant to how we would implement the approach in practice. With no elective nodes, isolated regional nodal relapse was 4% vs 8% (2/49 vs 2/25). Reassuring, although that does not establish the optimal elective volume (especially considering these patients went to surgery - ASTRO/NRG recommends inclusion of elective perineural coverage). Overall G3+ AEs were 70% vs 64% (33/47 vs 16/25). One G3 pancreatitis and one G2 small bowel obstruction were attributed to SBRT. The practical takeaway for me is to keep the target definition and prospective QA attached to the results, and to be precise about what the surgery and R0 numbers actually measure.

🔥 SANO 5-year results🔥 Esophagus cancer 👥 309 pts with cCR after nCRT 🔍 Surveillance vs surgery ✅ 5y OS: 50% vs 54% (HR 1.17) ⚠️ Distant mets: 46% vs 29% (P<.01) 💥 43% organ preservation Active surveillance remains a feasible strategy albeit we must recognize the DM risk despite no OS difference. Perhaps TNT, like D-FLOT ➡️CRT with subsequent AS for cCR may be an optimal next strategy to test. Improved response assessment strategies also needed #ESMO26

#ASCO26 The PROTEUS trial results are now online...buckle up as we wait to see the full presentation. This is going to be a trial that is likely highly controversial until the full results are published. Some may call this a homerun, others may call this the largest negative trial @ASCO 2026. 1/6: I am excited to share our multi-institutional F-SHARP phase I/II trial, now published in @JCO_ASCO! doi.org/10.1200/JCO-25-02730 There are limited prospective data defining the optimal management of intraprostatic radiorecurrence (IPR) after definitive RT for prostate cancer. We asked: 1. Is dose-escalated focal salvage high dose rate brachytherapy safe and effective for IPR? 2. Can Decipher (@Veracyte) identify patients with higher risk of recurrence with salvage ReRT that may benefit from therapeutic intensification? 🧵👇

Every cancer organ gets the same sequence: surgery is the standard, then trials show you often don't need it, then surgery becomes the salvage. Breast, larynx, limb, rectum, esophagus, mesothelioma. Bladder is next. Patients want their organs. 🧵

Thrilled to share that the The American Radium Society (ARS) Benign Committee has just published our new Appropriate Use Criteria (AUC) on Low-Dose Radiotherapy for the Treatment of Inflammatory Periarticular Disorders, covering conditions such as plantar fasciitis, shoulder tendinopathy, and greater trochanteric pain syndrome. As Vice Chairman of the committee, I am incredibly proud of the dedication, quality, and momentum our team has maintained over the past six months. A special thank you to Suzanne Russo and Austin Dove for their exceptional leadership throughout this initiative, and huge congratulations to Austin Kirschner, MD, PhD for leading this particular publication to completion!

Importantly this is modern data that with a spacer gel and SBRT at 2 years 80% of men remain potent. This is in a prospective RCT with patient reported outcomes.

Long-term PACE-A data at #ASTRO26 (van As LBA32, Yasar LBA10): the first randomized comparison of prostatectomy vs SBRT, 36.25 Gy in 5 fractions. These are contemporary surgical data: all prostatectomies were laparoscopic, 84% robotic. Using ≥1 pad/day, RP vs SBRT: • 2 years: 50% vs 7% • 5 years: 48% vs 8% Sexual function at 5 years: • EPIC sexual domain: 62→17 after RP vs 79→58 after SBRT • Median IIEF-5: 5 vs 19 Important caveats: as-treated analysis, approximately 60% PRO completion at 5 years, baseline sexual function imbalance, and enrollment of 123 of a planned 234 patients. Biochemical control looks similar through 8 years, but 13 events cannot establish equivalence. For shared decision-making, these are numbers I’d want patients to see. Every patient considering prostatectomy should have the opportunity to meet with a radiation oncologist before choosing treatment. #RadOnc #PCSM

I am pleased to share the results of our phase II randomized I-SABR study @ASTRO_org 2026 which demonstrate improved overall survival (OS) when adding four doses of immunotherapy to SABR in early-stage NSCLC. These exciting findings are associated with the persistent activation of cytotoxic T cells for up to 6.5 years. Further optimization of I-SABR approaches will guide future research and clinical practice.
Replying to @UTMDAnderson
Dr. Joe Chang shared six-year data from the Phase 2 I-SABR trial. He also shared that patients who received both nivolumab and SBRT treatment showed increased activity of cancer-targeting CD8+ T cells: spr.ly/6018BGWA4O (2/2) #ASTRO26 #EndCancer
