In healthy unanesthetized volunteers, norepinephrine increased arterial pressure but decreased the cerebral blood flow. Cerebral blood flow was correlated with cardiac output, not arterial blood pressure. Read more: ow.ly/fEfL50QUKcB

What patients fear most about operations
‘Will I wake up?’
#NAP7 report answers this.
-who will
-who is at risk of not
-what are the numbers?
Eg cardiac arrest during all surgery
1:3000
Fit & well, elective surgery
Cardiac arrest <1:10,000
Death <130,000
Frail patients
Cardiac arrest 1:1200
Death 1:2000

Have you ever had to do a needle decompression of a tension pneumoperitoneum in an unstable patient prior to the induction of general anaesthesia?

It shows that inexperienced providers (< 5% anaesthesia), more familiar with VL (<7% had used DL for more than 75% of previous intubations), do better with VL.
They lack skills with DL and fail to intubate on first attempt 1 in 10 times even with VL (which they’ve used more often).
This is not a death nail for DL as much as it is a dire warning that experience is key, even with VL, and that in inexperienced hands DL still has a high FPS.

When we published REGAIN, using a patient-informed primary endpoint of walking recovery at 60d, one of the most common critiques we received from the anesthesia community was that we didn’t focus primarily on a more proximal outcome, like “complications” 1/
Replying to @GreggWStone
3/3 Our suggestion: listen to the pt. They want to “live longer and live better”. New primary endpoint: Hierarchical death or time-integrated change in QOL (generic > disease-specific), tested with the WIN ratio. We plan on implementing this in an upcoming trial, if funded.
