The use of AI and healthcare extenders should be a platform for considering an entire revamping of medical education and how our healthcare is delivered. The critical issue is to learn how to learn, not just the facts, which has been a major emphasis of the pre-medical pathway as well as medical education. This ancient approach will need to change very rapidly.

This represents one of the most important advancements in safeguarding and improving the health of our nation. Randomized clinical trials are the cornerstone of rigorous, objective, evidence-based medicine. They provide the scientific foundation for transforming healthcare, improving outcomes, and helping Americans live healthier, longer lives. This is how we move medicine forward—not through speculation, but through evidence. Congratulations on this extraordinary achievement and its potential to transform the future of American healthcare! @EricTopol @mcuban @chamath @vkhosla @AHRQGov

Everyone fails. Anyone you see succeeding is only succeeding at the things you're paying attention to--I guarantee they are also failing at lots of other things. The people I respect most are those who fail well. I respect them even more than those who succeed. #principleoftheday

🚨BREAKING: Claude is officially a NATIONAL SECURITY THREAT and is BLACKLISTED from the ENTIRE defense supply chain >anthropic: no autonomous weapons, no mass surveillance, no exceptions >pentagon blacklists claude as an active national security threat >anthropic sues Federal court ruled 2–1 AGAINST Anthropic: "As Anthropic admits, the company encodes restrictions into Claude that prevent the model from performing tasks that Anthropic wishes to prevent." "On more than one occasion, these restrictions have stopped Claude from performing tasks requested by government users": >refused to perform "tasks that were appropriate in a national security context" >refused CDC queries on infectious disease prevention research >anthropic questioned claude's use in the military operations "Anthropic's Chief Science Officer explained how the company 'seek[s] to embed safety considerations directly into the model itself.'" "[Anthropic's] CEO explained how such training gives the model an 'identity, character, values, and personality' of its own, tethered to a 'constitution' developed to impose 'high-level principles and values' on Claude itself." "We have no reason to doubt that Anthropic manipulates Claude's function with noble intentions... BUT the statutory definition of a 'supply chain risk' turns on what Anthropic does, not why Anthropic does it." "In our Republic, it is the President and the Secretary of War who must determine how best to balance the competing risks. In doing so here, the Secretary DID NOT transgress any limits on his authority." ITS OVER. ANTHROPIC IPO IN SHAMBLES

Everything has happened before. Everything is repeated. But the mistake that had the biggest effect on me was in 1982. I had calculated that American banks had lent a lot of money to a bunch of countries that would not be able to pay it back. Mexico defaulted in August 1982, and while I was right about that part, I was completely wrong about the market. I thought the stock market was going to go down, but it went up because the Federal Reserve eased. I didn't just miss. I was publicly wrong and lost everything. I lost my company and all the people who worked for me—people who were like family. I was the only one left. I had to borrow $4,000 from my dad just to help pay my family bills. It was one of the most painful experiences I have ever had, but it was also one of the best. It completely changed my attitude about decision-making. It taught me to move from thinking "I'm right" to asking myself, "How do I know I'm right?" #RayDalio #Principles #Mindset #BusinessLessons

Regarding dilution due to crossover: TRANSFORM is not ISCHEMIA. These are asymptomatic, older, metabolically high-risk patients—not patients already known to have moderate/severe ischemia. In ISCHEMIA, despite much more advanced disease, only 8.7% of the conservative arm underwent angiography within 6 months. At ~8% crossover, a true 20% benefit is diluted only to ~19%. Hard to imagine TRANSFORM crossover approaching 25%. Its strength is testing whether CTA can identify and treat disease earlier—before symptoms and ischemia appear. #plaquedirectedtherapy @DLBHATTMD

A more open system would better serve all parties. With the rapid revolution in medicine and constantly evolving data, the most important skill is learning how to learn. In our current environment, however, the system often feels less focused on improving patient care and more on preserving existing structures. This creates artificial barriers to entering the field and can limit progress rather than accelerate it. #HealthcareReform #MedicalEducation
