IT’S NOT THE DOCTORS DRIVING UP HEALTHCARE COSTS!!! LOOK AT WHAT THESE SOULLESS, GRIFTING, CHARLATAN, MONEY WORSHIPPING, INSURANCE COMPANIES ARE DOING!!! THEY ARE LYING ABOUT DIAGNOSES TO MAXIMIZE BILLING!!! WHY ISN’T THE FEDERAL GOVERNMENT AUDITING THESE THIEVES AND TAKING THEM TO COURT?!?!!? @CMSGov, @SecBecerra, @FTC QUIT BEING INDUSTRY SHILLS AND DO YOUR GODDAMNED JOBS!!!
Squeezing blood from a stone instead of working to improve patient outcomes and experience seems to be the American way.

It’s not a technology barrier, it’s a perverse incentive barrier.
Hospital and multispecialty group administrations have big incentives to make filesharing as difficult as possible:
1) Outmigration moat: if it’s too easy to share films to outside physicians, patients might get a second opinion and stick with the outside doc. Making file sharing difficult makes it harder for patients to leave.
2) HIPAA enforcement paranoia: Admins believe that it is easy to get sued/fined if you accidentally share too much info (violating privacy), whereas no one has ever heard of getting sued/fined for sharing too little info.
3) Lack of feedback: If your healthcare group is bad at sharing films with physicians inside your group, those physicians will complain and admin has pressure to do something. But if you’re bad at sharing outside your group, the outside physicians have no way to get anyone to listen to their feedback.

Excellent thread and has significant implications beyond physician specialization and compensation
Just came across this tweet and it does a better, concise job of explaining the persistent, structural undermining of #valuebasedcare than most any other single source I've read.
The @AtriumHealth #MIHCP #CommunityParamedic summit is the largest #MIH centered professional conference and should be on your priority travel list
Great synopsis and some real action items
Things I'm thinking about while sipping a beer before flying out from #NAEMSP2023
1) There are so many brilliant minds in EMS. Agencies across the country and the world are pushing the industry forward.
Certainly critical for physicians, but understanding regulatory burden and not falling prey to unnecessary "CYA" complications (that tend to compound) is key to improving access to care and decreasing barriers

More often than not numerous risk factors are inaccurately oversimplified into a patients race which leads to (a) premature diagnostic closure and (b) false attribution of biological significance to race
Big steps forward in NJ for #paramedicine and #MIHCP
New Jersey has passed a law to license #paramedics and enshrine mobile integrated health in the #ems scope. This is great work. Will NC and others also follow such progress? www-nj-com.cdn.ampproject.or…

What's old is new and the opportunities for multidisciplinary teams, #MIHCP , to make a substantial impact in the future of primary care can not be overstated
