I see 'problem' is right up there too. So many times people jump to the assumption of problem without actually understanding it or they suggest a problem that their solution fits - wasting so much time and effort.

Very often in healthcare, we make a massive effort to improve our delivery performance by changing our processes and practices. We should probably start much higher up in our knowledge structure by considering our paradigm - our beliefs about how to make change happen. Mistake proofing: this small catch prevents the shuttle flying on the loom when the thread has snapped. Such a simple concept preventing a defect in the fabric. Improvement doesn't need to be complicated 👍
