Our framework and some ideas along with Zeshuan Khawaja and @TimShowalter1 for scaling #AI models across the @CleClinicMD cancer service line and a case study in the form of our ongoing collaboration with @viz_ai (sponsors of the perspective article for transparency). @jamecancerdoc @FaizAnwerMD1

One of the clearest signals from last week's AI Vizionaries Summit: clinicians don't want to go back to their pre-AI ways of working.
The conversation wasn't about whether clinicians will adopt AI. It was about what adoption actually looks like once AI is already part of the workflow. @Viz_ai and @HLTHEVENT invited clinicians, health system leaders, and pharma executives to sit together in the same room to work through exactly that, from identifying the right patients earlier to supporting the calls clinicians make every day.
What the clinicians said:
- The best AI works as a safety net
- AI earns its place in the workflow
- Scale is what turns a good tool into better care
What pharma said:
- AI has moved from the back office to the point of care
- The hard part is implementation
- Workflow AI belongs in core pharma plans
Trust, workflow integration, clinical value, alignment across stakeholders: that's what kept coming up, and none of it comes from the technology alone.
Thank you to @HLTHEVENT, our panelists from @SanofiUS , @Novartis, @Merck, @CooperHealthNJ, @ClevelandClinic, @JeffersonUniv, and Integrated Medical Professionals.
Stay tuned for the full report from our Summit, and meet us at #HLTH2026 in Las Vegas, November 15-18.

In a phase 1 study of anito-cel, a CAR T-cell therapy, for relapsed or refractory myeloma, all 38 patients had a response. Severe cytokine release syndrome and immune effector cell–associated neurotoxicity syndrome were rare. Full study results: nej.md/4ArvK6r

ALT A graph from an Original Article published in the New England Journal of Medicine article titled "Phase 1 Study of Anito-cel, a D-Domain BCMA CAR T Cell for Refractory or Recurrent Myeloma." The graph shows progression-free survival among all patients who received anito-cel. The NEJM identity sits at the bottom.

I agree with the diagnosis in the article, but there’s a paradox it misses: academic medicine helps create the very problem it laments
Yes, we underpay people relative to their value, then moralize about it. Leave for pharma? “The dark side.” Consult for industry? COI. Move to start up? Too aligned with payers. Negotiate aggressively? Not mission-driven enough or even worse have to answer to non-physician administrators
Law, business, tech and politics accept that talented people seek compensation, influence, ownership, and value
Maybe the problem is that we’ve built a culture where wanting to be fairly rewarded is treated as a character flaw or selfish

Replying to @ClevelandClinic
@ClevelandClinic has been a Viz.ai partner since our early work in stroke care coordination. Since then, the partnership has expanded into cardiology, deeper EHR integration, and new work in oncology and pulmonary care.
Today at Cleveland Clinic’s AI Summit, Viz.ai’s Dr. @TimShowalter1 joined Cleveland Clinic leaders Dr. Zeshaun Khawaja @BustaClot and Dr. @LouisWilliamsMD for a panel on AI at Scale: How Health Systems Are Turning Promise into Practice.
The panel discussed what it takes to move beyond individual AI deployments towards an enterprise approach:
→ Start with the clinical problem
→ Bring clinical, IT, data and operational leaders together early
→ Build governance, infrastructure and integration that reduce friction as AI expands
→ Integrate into clinical workflows
→ Measure success by the impact on patient care, operations and the clinician experience
Cleveland Clinic is building on what works and extending the value of clinical AI to more clinicians, specialties and patients. It’s a model for how sustained partnership can help health systems move from individual use cases toward an enterprise approach.
Thank you to Cleveland Clinic for bringing healthcare leaders together and to Dr. Khawaja and Dr. Williams for sharing their experience.

Stepping into a technology facing role as a clinician is challenging. I discussed issues related to #AI implementation with @BustaClot,
@TimShowalter1 and @viz_ai at the @ClevelandClinic Healthcare AI summit. Here are my thoughts on three quick questions to ask yourself about the scaling of #AI if you’re a clinician responsible for digital strategy at an academic cancer center.
2. What could this “look like” in a cancer center?
For marginal cost to come down, a good start might include the following: a smooth governance structure tiered to risk level of individual models, a portfolio-based strategy, fully developed data infrastructure and stack that (EDW, data lake, etc) to reduce friction in model deployment, and a seamless workflow for implementation and for broader operational deployment of successful pilots.
Great resources available, especially on governance from @CHAI_nonprofit
chai.org/workgroup/responsib…
3. How do you work with technology experts to get there?
Mutual understanding should prevail. Clinicians in these roles should invest in learning how common models work. You don’t need a degree in computer science but you do need to know the lexicon and general concepts. Technology experts and data scientists should be included early on when addressing a clinical problems. Finally, responsibility for long term quality assurance should be shared.
Lots of great resources out there.
Highly recommend asking your favorite #LLM how it works. Then prompt it to describe typical layers of data architecture in a typical cancer center.

Excited for the @ClevelandClinic Healthcare AI Summit tomorrow, August 28, at the InterContinental Hotel Cleveland!
With more than 1,000 participants joining in person and virtually, this has become one of the largest AI in healthcare conferences focused on clinicians and patient-facing healthcare teams. clevelandclinicmeded.com/liv…
Grateful to our sponsors for their support and to our outstanding organizing committee for their remarkable work in making this event possible. Looking forward to a day of learning, collaboration, and discussion on how AI can help us deliver better care while keeping patients at the center of everything we do.

Moving from ad-hoc pilots to coordinated scaling of #AI initiatives is one of the cardinal challenges facing health systems in the face of rapid evolution of new models and new applications. Join us live or virtually as we explore this topic on Friday at the Cleveland Clinic AI Summit. @viz_ai @BustaClot @jamecancerdoc @ClevelandClinic @CleClinicMD

Median OS finally reached with Dara-Rd in ASCT-ineligible ND multiple myeloma…
… and it’s >7.5 years! Gone are the days of mOS 3-5 years, and in are the days of Dara-Rd functional curing many older patients 👏
Congrats #thierryfacon @myelomaMD @szusmani and all co-authors!
Final MAIA analysis: D-Rd reached median OS of 90.3 months (HR 0.67 vs. Rd) after 89.3 months of follow-up in transplant-ineligible NDMM. A tough number to beat - will quadruplet regimens have to work hard to surpass it? doi.org/10.1038/s41375-026-0… #MM #mmsm

Great work by @johnhanna0, Sandy Mazzoni and team. Always good to see data behind what we observe day to day in clinical practice.
#Myeloma Paper of the Day: Limited duration (≤3 months) of dexamethasone in newly diagnosed AL #amyloidosis (w/ daratumumab-based regimen) results in higher 6-month CR rates (66.7 vs 30.4%, p<0.001) and reduced toxicity, similar organ responses: pubmed.ncbi.nlm.nih.gov/4256…. #alsm

I am very optimistic about recent changes in the management of multiple myeloma and think the concept of cure is real. With this being so we are moving to an era where the design of deescalation studies is going to be important
💬 Editorial by @JAMAplusTrials Editor in Chief Alison Huang, MD, MAS, Mara Schonberg, MD, MPH, and JAMA Internal Medicine Deputy Editor Deborah Grady, MD, MPH: Deescalation, discontinuation, and deimplementation trials increasingly evaluate whether reducing or halting interventions—such as medications, screenings, or therapies—provides comparable or improved outcomes for patients, especially when current practices are suspected of limited value or potential harm.
