The hardness of medical school is not really about the intellectual difficulty but about the volume of stuff you have to recall and that's why many struggle.
For instance, over the course of a 6 to 8 week posting, you would be required to memorize about 30% of the content of 3 or 4 textbooks.
Then you're assessed on this.
Next 2 postings, you're then asked to memorize the remainder of those textbooks and you're assessed on them.
As if that's not worse enough, finally there are these exams called MBBS professional exams in which you're then assessed on the entire content of the textbooks in different modalities.
You'd write objective, theory, picture test, osce practical and Viva.
So yeah medical school can be hard.

There is something called Pharmacoencomics.
The pharmacist/lecturer who taught us pharmacology back then in school taught us this as an intro to pharmacology and it is one lecture I will never forget.
I don't think there is any doctor who will tell a patient that it must be Rocephin or nothing else without first looking into the financial pocket of their patient and trying cheaper options.
We start our clerking with biodata, where we ask patient for level of education and occupation, this one of the reasons we do it.
