📢 Results of "OnScene" trial, nationwide prehospital helicopter ECPR program in 🇳🇱, are out on @NEJM + have been just presented at #ReSS26
🚁 In young adults with refractory #OHCA who were attended by HEMS equipped to deliver prehospital #ECMO (intervention group, 144 patients) vs those attended by HEMS without ECPR capability (conventional group 87 patients), incorporating ECPR was associated with improved survival to discharge vs conventional care. In intervention group 40% survived until hospital discharge (vs 20%). At 6 months, a favourable neurologic outcome reported in 36% in the intervention group (vs 13%). Full results at

A historic day for Resuscitation Science at @AHAScience #ReSS2026.
ON-SCENE is out in @NEJM — nationwide HEMS ECPR in the Netherlands, cannulation on scene.
40% vs 20% survival to discharge. 36% vs 13% CPC 1-2 at 6 mo. 45 vs 77 min to ECMO flow.

I analyzed the current #ESC2026 Heart Failure Guidelines 2026 for conflicts of interest. My conclusion: guideline development should demand maximum independence. Industry can and should support research—but it must never determine recommendations. #GuidelineIndependence

Two randomized trials of FFR estimates from angio images compared to traditional wire based FFR out in NEJM
Both claim non-inferiority in a composite of of death/MI/late revasc
But neither counts the ~9% increase in rate of PCI with the new method
If total PCIs were counted, both of these would fail to show non-inferiority - i.e. simulations of ischemic testing are *inferior* to measurement!
The corresponding editorial goes further with a long discussion of CT simulations of ischemia, as if this is the standard non-invasive approach, when stress imaging has served this role for decades and continues to be the dominant approach.

Today marked an important step forward in US resuscitation science!
The Center for Resuscitation Medicine’s MMRC completed the first Helicopter EMS–facilitated extracorporeal cardiopulmonary resuscitation (ECPR) in the United States!

After #PFO closure, continuous implantable cardiac monitoring revealed a high incidence of atrial fibrillation: 19% at 30 days and 28% at 1 year.
Most episodes are asymptomatic and paroxysmal, with a significant proportion occurring beyond 30 days.

TOP-HAT is top notch!
Here is a nice little hack to tackle persistent bleeds after Perclose + wire removal.
The technique was recently described by Ahmad et al in @ccijournal 👉🏼 doi.org/10.1002/ccd.70409
88 y/o man s/p Impella assisted LM PCI. We thought we had successful closure of the 8 Fr access with Preclose. Alas!
TOP-HAT worked like a charm! No more bleeding in <1 min!

Left ventricular recovery during VA #ECMO?
🫀 pulse pressure directly proportional to SV/inversely proportional to arterial compliance = indirect marker of LV pump function
🩸 PP < 15 mm Hg strongly correlates with severely reduced native CO (< 1 L/min)
🩸 PP < 20 mm Hg associated with decreased survival
However PP alone still be little informative as it may be the same for remarkably different hemodynamic states
🫀 here, proposal of modified cardiac power output incorporating PP vs MAP, to avoid poor or absent pulsatility and/or ineffective unloading biasing its interpretation in reflecting efficacy of organ perfusion

Our beliefs about LAAC should have been pessimistic going into the trial: CLOSURE AF finds extremely pessimistic data—which only strengthen these priors.
