Allo-transplant is still the only cure in myelofibrosis — and still carries 20–30% TRM, for the minority who are eligible.
So what happens when CALR-directed therapies arrive?
New review out today in Bone Marrow Transplantation, open access. We work through the landscape — peptide vaccines, the Fc-silenced mAb INCA033989, bispecifics, a first-in-class ADC, and RPG4 CAR-T — and ask what each does to the transplant decision.
Our take: signalling blockade is the most clinically mature, but blocking mutCALR–TpoR likely won’t clear the clone. Cytotoxic approaches have the mechanism for that depth, but no clinical data yet. High-risk patients should still go to transplant. The near-term opportunities are earlier disease, transplant-ineligible patients, and post-transplant MRD.
One unsolved problem: there’s no validated definition of progression on a disease-modifying immunotherapy. IWG-MRT was written for cytoreduction, not this.

In a trial involving adults with Ph-positive acute lymphoblastic leukemia, 24-month event-free survival was 86% with the TKI–immunotherapy combination ponatinib–blinatumomab and 67% with imatinib plus chemotherapy. Full phase 3 GIMEMA ALL2820 trial results: nej.md/4scorw0
Editorial: Chemotherapy-free Treatment of Ph-Positive ALL nej.md/3VIVUC8

ALT A graph from an Original Article published in the New England Journal of Medicine article titled "Ponatinib–Blinatumomab or Imatinib–Chemotherapy for Adult Ph-Positive ALL.” The graph shows event-free survival (the primary end point) in the intention-to-treat analysis among patients in the intervention group (who received ponatinib plus blinatumomab) and in the control group (who received imatinib plus chemotherapy). The trial name, GIMEMA ALL2820, is in parentheses after the title. The NEJM identity sits at the bottom.

Fascinating lessons from the EVITA ph2 trial (published in Cancer) evaluating oral Vitamin C in CCUS & lower-risk MN:
📌 The Endpoint Trap in Precursor States!!
Primary endpoint (median clonal growth rate over 12 m) showed no difference between VitC and placebo (p = .70).
💡 Beyond Clonal Growth:
- Fewer serious adverse events (33% vs 57%)
- Shifted inflammatory cytokine trajectories
Exploratory OS showed a ! signal (HR 0.35, p = .0025), it is not powered to look at OS, the most important lesson.

Amazing topic!
Delighted to be invited again to speak on one of my favorite topics: "Building TRUST in Clinical Trials" and eradicating barriers to #clinicaltrialenrollment at the @SylvesterCancer Design & Management of Clinical Trials Course
Some pearls:
▶️Ask every eligible patient
▶️Earn trust before the trial, show up where pts are
▶️Match language and culture
▶️Use navigators to ensure retention from day one

Chemotherapy and ionizing radiation are known to promote clonal hematopoiesis and its progression to leukemia. By inducing temporary quiescence of hematopoietic stem cells during external stress might we mitigate CH expansion? We address this through a collaboration with @AbdelWahablab @PuZhang09980432: nature.com/articles/s41588-0…. Excited to share this and some thoughts from a recent meeting in Hiroshima which puts this work in a broader perspective.
