مريضة تعاني من تضيق شديد في الصمام المترالي مما تسبب في قصور القلب الاحتقاني والتنويم المتكرر في المستشفى، تم علاجها في مركز سعود البابطين لطب وجراحة القلب بالدمام في إجراء نادر بالتدخل المحدود عن طريق القسطرة من خلال زرع صمام مترالي داخل حلقة جراحية وذلك بعد تعديل الشرفة الأمامية للصمام وتم إغلاق الثقب الأذيني بعد زرع الصمام.

In patients undergoing TAVI, the occurrence of coexisting severe aortic stenosis and coronary artery disease with high-risk/complex features is rare (2.83%) but has demonstrated an increasing trend over the years. Performing high-risk PCI concomitantly to TAVI is associated with an increased risk of in-hospital vascular and bleeding complications. In conclusion, in this subset of high-risk patients, a strategy of staged PCI before TAVI appears associated with the best outcomes. #EIJBestOf

Another SIM PUB online today presented #EuroPCR
The multicentre HERA-TAVI registry compared contemporary intra-annular and supra-annular self-expanding transcatheter heart valves in routine transfemoral TAVI practice.
At 1 year, both platforms demonstrated similarly low rates of death, disabling stroke, and heart failure hospitalisation, together with sustained haemodynamic performance and low rates of clinically relevant PVL.
Distinct procedural trade-offs were observed, with higher pacemaker implantation following IA-SEV and higher bleeding rates following SA-SEV implantation.

From watchful waiting to early TAVI, the review analyses recent randomised trials including EARLY TAVR and EVOLVED, highlighting the balance between preventing heart failure hospitalisations and procedural/lifetime risks.
