🚨 BREAKING EXCLUSIVE: GLP-1 ACCESS
Massachusetts General Hospital was ground zero for some of the foundational science that gave us GLP-1 medicines, and later became a founding member of Mass General Brigham.
Now, beginning Jan. 1, affected MGB employee health plans are REMOVING GLP-1 obesity drugs from the traditional pharmacy benefit, while a CVS letter obtained by On The Pen shows other obesity medications are being removed as well.
MGB confirmed the change directly to On The Pen and says eligible employees will instead receive income-tiered financial assistance through an HRA.
The institution that helped teach the world obesity is biology is now at the center of a fight over what access to treatment should look like.

The battle over America’s peptide market is heating up.
The Partnership for Safe Medicines wants the FDA to block compounding of seven peptides, while medical organisations, pharmacies and telehealth companies are meeting White House officials over expanded access.
PSM has also been buying injectable peptides from licensed pharmacies and questioning whether those suppliers are operating legally.
Meanwhile, Bloomberg reports that the proposed FDA policy could introduce mandatory adverse-event reporting.
So who’s pushing for access, who’s trying to stop it, and what does each side stand to gain?

So @TylerShultz_ as a Theranos whistleblower. And he wrote the piece you need to read about ‘You Can See Everything.’
Elizabeth Holmes is still lying, he says. And the filmmakers, while creating a great piece of art, fall short on looking outside the walls of her apartment to document that.
One key detail: "My wife had to squeeze my arm to prevent me from jumping out of my seat in the theater and screaming, THAT IS NOT THE DEVICE."
Again, that device Holmes showed? It's not the one we're talking about!!!!
Having seen the film, I basically agree with Tyler's take.

EXCLUSIVE: Elon Musk came to Donald Trump with the plan to throttle Ukraine’s use of SpaceX’s Starshield satellite internet network, sources have told the Telegraph.
The tech billionaire made the proposal during the US President’s row with Volodymyr Zelensky over strikes on Russian oil refineries.
‘The reality is that they were both in it,’ a senior source said of Musk’s and Trump’s involvement.
But after a night of carnage in Kyiv caused by the slowdown, Musk was alarmed about the possibility of adverse consumer reaction in Europe.
The SpaceX boss then proceeded to tweet, blaming the US administration, and ultimately, President Trump for authorising the slowdown.
‘He tweeted out that he was innocent and that it was the US administration that did it,’ the source said.

I saw the Elizabeth Holmes documentary 'You Can See Everything' last night.
Everyone's debating whether she's a liar. That's not the story.
The film is deceiving you in real time & most of you won't notice.
I wrote about it 👇

🚨A positive step worth following:
FDA has submitted an “Interim Access Policy for Compounding Using Certain Peptides” (HHS/FDA Notice, RIN 0910-ZD94) for review. It was received just yesterday and is currently pending review.
This looks to be the interim pathway that would allow compounding pharmacies to compound certain peptides (the ones PCAC recommended in July) under a valid prescription while the longer formal rulemaking process continues. It has not been issued yet, so nothing has changed legally as of now, but it is a concrete move toward patients being able to access those peptides through regulated compounding pharmacies. reginfo.gov/public/do/eoDeta…
So very exciting for patients and providers who want peptides, but to receive them via prescription and licensed pharmacy, to be used under doctor supervision.
I’ll be watching this closely. Stay tuned for more updates. Strive is gearing up to compound these as soon as we’re allowed to.

❗️Last night, the operation of Starlink terminals in Ukraine was restricted.
Connectivity began to drop when moving at speeds exceeding 160 km/h. As a result, a number of critical missions were disrupted.
SpaceX likely took these measures at the request of the Trump administration. Also last night, when Kyiv was attacked by ballistic missiles, Ukraine did not receive intelligence regarding their launch— a failure that could have led to higher casualties.
Ukraine did not attack Russian oil refineries, yet Trump has already begun imposing sanctions on my country. Meanwhile, sanctions against Russia have been eased.

The man arrested for planting a secret recording device at OpenAI's headquarters is a Venture Partner of a VC firm alongside the son of a senior Saudi intelligence official:
FDA reportedly planning “enforcement discretion” on PCAC-recommended peptides - this is no safe harbor for 503a pharmacies who choose to compound and dispense these peptides.
7th Cir. Franco v. Chobani case was not just important for the Lilly / Reta appeals case but also here - that decision confirms that federal agency non-enforcement doesn’t rewrite unapproved-drug status or preempt state consumer lawsuits or class action lawsuits.
Limited 503A non-enforcement ≠ authorization for RUO sellers, non-compliant distribution, or therapeutic claims. Risks remain to RUOs and compounders.

I guess happy days are here again for large health insurers.
United beat and raise third quarter in a row and Humana at all time highs.
The actuaries are allowed back at the Christmas parties again this year, I suppose.
Will the MA crisis of the last 2 years lead to any structural and favorable changes for patients or providers? Nope.
South Asians have up to 6× higher T2DM risk, often diagnosed in younger and leaner vs. other populations.
New (small) mechanistic study using deuterium-labeled water + MR scans shows South Asians have higher de novo lipogenesis driving excess liver fat and insulin resistance vs matched Europeans.
Use of GLP-1s that cut liver fat may be especially critical in this group.
