Is portal vein thrombosis more than a clot?
In CLEARED, PVT carried higher 30-day mortality despite lower MELD-Na. This editorial argues PVT reflects structural disease of the portal vein, which may explain why anticoagulation often disappoints.

For patients undergoing upfront surgery for colorectal liver metastases, postsurgical 🧬 ctDNA identifies who will derive overall survival benefit from (m)adjuvant chemotherapy, and who can safely avoid it.
🧬MRD (+): Adjuvant chemotherapy significantly improved both disease-free survival and overall survival.
🧬 MRD (-): Patients experienced excellent long-term survival regardless of whether they received chemotherapy or just observation.

As systemic therapies continue to improve patient outcomes, we as surgeons are increasingly confronted with a question that would have seemed unrealistic just a decade ago:
Which patients might actually benefit from surgery despite metastatic disease?
Our new international, multicenter study led by Orlando Jorge M Torres and Fabrizio di Benedetto—just published in the @IJSurgery —explored this exact question in patients with synchronous, liver-only metastatic PDAC. While our findings certainly do not provide all the answers, they do add real life evidence that carefully selected patients with favorable disease biology represent a distinct subgroup worthy of deeper investigation.
For me, the most profound takeaway from this work is that progress in pancreatic cancer will not come from surgery, medical oncology, or technology in isolation. Real advancement relies entirely on a better understanding of tumor biology and a thoughtful, multidisciplinary integration of every tool we have.
I am incredibly proud to have contributed to this collaboration. My sincere thanks to my colleagues across the @islsstudygroups network @BurcinEkser who continue to challenge the status quo in pancreatic cancer care.
