The Health Service Ombudsman wants end of life care prioritised in the single patient record, with advance care plans, ReSPECT forms and DNACPR decisions "consistently accessible and editable across all care settings". Editable is the key word. blackpear.com/news/single-pa…

Four lessons from six years of SIDeR+ in Somerset: 1. Structured, real-time data beats documents 2. A shared record must be one you can write to 3. Adoption is the work 4. Patients don't stop at boundaries What they mean for the single patient record: blackpear.com/news/single-pa…

The idea that our diet can be an important part of mental health is gaining ground How many of us have patients who have more energy or feel less anxious on a diet with less sugar in it ?
🧠 Metabolic psychiatry is growing, and now there’s a new global network built for the people advancing it. @KirkNylen and @GeorgiaEdeMD join @bschermd to introduce MetPsy and explain how clinicians and researchers can get involved.

Connecting shared care records is the part that's easy to count. The harder question is whether clinicians use what they see when decisions are made. I've written about what Somerset has taught us, and what it could mean for the single patient record: blackpear.com/news/single-pa…

The single patient record's first milestone is improved access for at least 80% of patients by March 2027, through existing shared care records. Our CEO @djehring on what six years of SIDeR+ in Somerset suggest about making that access count: blackpear.com/news/single-pa…

Fred Kummerow told America in 1957 that the fat in margarine was turning up in the arteries of the dead. The government acted on it in 2015. He was 100. He was a biochemist at the University of Illinois, and what he had done was simple and slightly grim. He got hold of tissue from people who had died of heart disease and looked at what their arteries were made of. They were full of trans fats, a kind of fat that barely exists in nature and came, in 1957, from one place: the hydrogenated vegetable shortening and margarine the country had just been told to eat in place of butter. He published it in Science. He was 43. What followed was a career spent being ignored by experts. The oil industry had friends on every committee. His funding grew thin. In 1968 he talked a panel of the American Heart Association into warning about trans fats, and the warning was pulped and reprinted without it after the shortening makers saw a draft. Kummerow kept working. He kept his laboratory into his nineties. And he kept eating, quite deliberately and in front of journalists, the diet he had been told would kill him. Eggs cooked in butter. Whole milk. Red meat. He had looked inside the arteries himself and he had not found breakfast in there. In 2009, aged 94, he filed a formal petition asking the Food and Drug Administration to ban artificial trans fats. The law gave the agency 180 days to answer. Four years later it had not, so in 2013, at 98, he sued the United States government. Three months after that the FDA announced it agreed with him. The ban was confirmed in 2015. The gap between his paper and the decision was 58 years, which is longer than most of the people who ate the margarine got. He died in 2017, at 102. And he spent those last years on a second warning, which has been received exactly as the first one was. The problem, he said, was never the cholesterol in the egg. It is the vegetable oil in the fryer, heated and reheated until it oxidises, and then eaten. On the record so far, that one is due to be acted upon in about 2070.

Insulin resistance and its twin sister hyperinsulinaemia probably account for 80% of my work as a doctor It predates T2D by about 10 years Critically it responds to a low carb diet more than a low fat diet Please look at our review published here mdpi.com/2072-6643/18/16/266… @BenBikmanPhD @BraciaRodzen @ketoproject1

Such an interesting study. Participants all lost about 10% body weight on three very different diets BUT what happened in terms of cardiometabolic measurements was VERY DIFFERENT, LOOK at the triglycerides and liver insulin sensitivity, so much better on a ketogenic diet I feel this is because diets express themselves via hormones like insulin. A calorie is not a calorie, it depends! cell.com/cell-metabolism/ful…

The Children's Mental Health Resource Center is recruiting for the next cohort of its longitudinal ketogenic therapy study in young people ages 6 through 20. The cohort begins September 1, 2026, and CMHRC needs at least twenty participants. When they contacted me, only eight had completed the screening and enrollment process. This is one of those moments when everyone who says we need better evidence in metabolic psychiatry has to understand what producing that evidence actually requires. It requires families willing to participate and we are going to help them find them. Please share this post far and wide in groups and among your professional contacts. The first fourteen days of the study are completed without changing the diet so that the research team can establish a baseline. Participants then follow a ketogenic food plan for two years while tracking symptoms, food intake, blood glucose, and ketones. Assessments are completed four times each year. After the active portion of the study ends, families meet with the research team four times a year for another five years. This is a meaningful commitment because the question being studied is larger than whether someone can produce ketones for a few weeks. The researchers want to know whether families can implement ketogenic therapy at home, whether symptoms change, and what happens over time. The study does not expect perfection. This is important. A family does not invalidate the research by having difficulty. Difficulty is part of the research question. Honest information about adherence, symptoms, food challenges, and periods outside ketosis helps investigators understand whether ketogenic therapy is feasible in ordinary life. Even hospitalization does not automatically remove someone from the study. The program continues to support the caregiver and helps the participant return to the food plan after coming home. Families should not decide on their own that they are ineligible. The study is recruiting young people ages 6 through 20 who live primarily in the United States and have a mood disorder such as bipolar disorder, depression, or disruptive mood dysregulation disorder. CMHRC also welcomes interest from families whose child has symptoms that may be consistent with a mood disorder but does not yet have an official diagnosis. Young people already receiving therapy or psychiatric care may participate. The interest survey begins the screening process. It does not guarantee enrollment or obligate the family to participate. cmhrc.org/keto-long-study/ Questions can be sent directly to [email protected].
